后极性白内障术前Pentacam检查发现后囊破裂的研究

摘　　要：目的探讨Pentacam眼前段测量分析系统对后极性白内障后囊破裂者术前晶状体后极部形态的观察，并分析其对手术的意义。方法后极性白内障3例（3眼），术前均行散瞳后裂隙灯显微镜检查和Pentacam检查，本组患者均行2．2mm切口超声乳化联合人工晶状体植入术，术中证实3眼后囊破裂。回顾性分析后极性白内障在Pentacam术前检查中后囊破裂的表现。结果在本组中，术前Pentacam检查发现3眼晶状体后极部局限性强反光团块状突破后囊水平，术中证实3眼后囊破裂，其中2眼行后囊连续环形撕囊，人工晶状体顺利植入囊袋内，另1眼行前部玻璃体切除术，人工晶状体植入睫状沟内。结论Pentacam检查和裂隙灯显微镜检查都可以观察后极性白内障晶状体浑浊的情况，但Pentacam检查可以发现后极部白内障术前的后囊破裂，提前做好术前准备，减少术中并发症的发生。Objective To investigate the morphology of posterior pole in eyes with posterior polar cataract combined with posterior capsular rupture using Pentacam anterior segment measurement analysis system before phacoemulsification surgery and to analyze the significance of this examination for cataract surgery. Methods Three eyes of 3 patients with posterior polar cataract were enrolled in this research. All of them were examined using slit lamp microscopy and Pentacam before the operation. Three patients underwent phacoemulsification with 2.2ram corneal incisions and intraocular lens （IOL） implantation, and during the surgery these patients were confirmed with posterior capsule rupture. The morphology of posterior polar rupture in posterior polar cataract was analyzed retrospectively using pentaeam examination. Results The local strong reflective mass in posterior pole of the lens breaking through the level of the posterior capsule was observed by the Pentaeam examination before the operation in three eyes. The posterior capsular rupture was verified during the operation in all eyes. Two eyes underwent continuous curvilinear posterior capsulorhexis, and the intraocular lenses were implanted in the capsular bags. The anterior vitrectomy was performed in one eye, and the intraocular lens was fixed in the ciliary sulcus. Conclusion Both Pentacam examination and slit lamp microscopy can be used to assess the opacity of posterior pole in eyes with posterior polar cataract. However, the Pentacam examination can detect posterior capsular rupture before the operation which could be advantageous for preoperative preparations and reducing the intraoperative complications.
