摘　　要：目的检NgL腺癌患者血清可溶性细胞间粘附分子-1（soluble intercellular adhesion molecule- 1, slCAM- 1 ）表达水平，探讨血清sICAM-1与乳腺癌临床病理学指标的关系及其临床意义。方法用酶免疫吸附方法（ enzyme linkedimmunosorbentassay, ELISA）对61例乳腺浸润癌患者和23例乳腺良性肿瘤患者外周血中sICAM-1的水平进行定量测定，并收集患者临床资料进行统计学分析。结果①乳腺浸润癌组患者血清sICAM1表达水平（1389．46±648．91ng／m1）显著高于乳腺良性肿瘤组（879．72±378．62ng／m1）（t=4．448，P〈0．001）。伴淋巴结转移组sICAM-1显著高于无淋巴结转移组（P〈0．001），Ⅲ～Ⅳ期乳腺浸润癌患者血清sICam-1水平显著高于Ⅰ-Ⅱ期患者（P〈0．05）。②ROC曲线分析血清sICAM-1水平1186．87ng／ml是区分乳腺浸润性癌和乳腺良性肿瘤的最佳临界值；sICAM-1水平1257．38ng／ml是判断乳腺浸润性癌是否发生转移的最佳临界值。③血清sICAM-1表达水平与病理分级、年龄、肿瘤大小、乳腺癌免疫组化指标ER、PR、Ki67及HER-2均无相关性（P〉0．05）。结论血清sICam-1可作为乳腺癌的血清学观察指标，对判断肿瘤发展程度、转移状态及复发状况有较好的临床意义。Objective To detect the expression level of serum soluble intercellular adhesion molecule - 1 （slCAM - 1 ） in pa- tients with breast cancer, and to explore its relationship with clinical pathological indexes of breast cancer as well as its clinical sig- nificance Methods Enzyme - linked immunosorbent assay （ELISA） was used to detect the serum concentration of sICAM- 1 in 61 patients with invasive breast carcinoma and 23 patients with benign breast tumor. And the clinical data of all the patients were collected to conduct statistical analysis. Results ①The expression level of serum sICAM- 1 was significantly higher in the invasive breast carcinoma patients than in the benign breast tumor patients [（ 1,389.46 ± 648.91 ） ng/ml vs. （879.72 ±378.62） ng/ml, t = 4. 448, P〈 0. 001]. The expression level of serum SICAM 1 was significantly higher in the breast carcinoma patients with lymph node metastasis than in those without （P 〈 0.001 ）. The expression level of serum SICAM - 1 was significantly higher in patients with stage Ⅲ - Ⅳ invasive breast carcinoma than in patients with stage Ⅰ- Ⅱ invasive breast carcinoma （P〈0.05）. ②CK; curve analysis indicated that the optimal cut - off value of serum sICAM 1 for distin- guishing metastasis invasive breast carcinoma from breast benign tumor was 1,186.87 ng/ml, and the best cut - off value of serum sICAM - 1 for judging the metastasis of invasive breast cancer was 1,257.38 ng/ml. ③There was no significant correla- tion between the expression level of serum sICAM- 1 and pathological classification, age, tumor size and immunohistochemieal indexes of breast carcinoma, including ER, PR, Ki67 and HER - 2 （P 2〉 0.05）. Conclusions Serum slCAM - 1 can be used as a dynamic index of breast carcinoma, and it has a good clinical value in judging tumor development, metastasis and recur- renee.
