低位直肠癌患者行保肛手术治疗的效果评价

摘　　要：目的探讨低位直肠癌患者行保肛手术治疗的临床效果。方法选取我院2010年7月至2012年6月收治的94例低位直肠癌患者,随机分为两组。对照组行Miles术,观察组行保肛术,比较两组围术期情况,术后并发症、复发率、生存率及满意度。结果两组手术时间、恢复普食时间比较无统计学差异(P>0.05);观察组的出血量、术后排气时间、住院时间均显著短于对照组(P<0.01);观察组局部复发率6.38%低于对照组的10.64%(P>0.05);观察组并发症发生率14.89%低于对照组的36.17%(P<0.05),观察组3年生存率91.49%高于对照组的74.47%(P<0.05);观察组总满意度70.21%高于对照组的48.94%(P<0.05)。结论低位直肠癌患者采用保肛手术的疗效明显,可改善患者预后,患者满意度高,可广泛推广。Objective To probe into the clinical effect of anus preserving operation for patients with low rectal cancer. Methods94 patients with low rectal cancer were admitted in our hospital from July 2010 to June 2012 were selected as the research object,and these patients were randomly divided into two groups： Patients of the control group were treated with the Miles surgery,while those of the observation group were given anus preserving surgery. Peri- operative conditions,postoperative complications,recurrence rate,survival rate of both group as well as their degree of satisfaction were compared. Results There was no statistical difference in the operation time and time of returning to the routine diet between the two groups（ P〈0. 05）,the amount of bleeding,postoperative exhaust time,hospitalization time of the observation group were significantly shorter than those of the control group（ P〈0. 01）. The partial recurrence rate of the observation group was 6. 38% which was lower than 10. 64% of the control group（ P〉0. 05）,in addition,the complication incidence of the observation group was 14. 89% which was lower than 36. 17% of the control group（ P〈0. 05）,3- year survival rate of the observation group was 91. 49% which was higher than the control group（ 74. 47%,P〈0. 05）,the total satisfaction rate of observation group was 70. 21% which was higher than the control group（ 48. 94%,P〈0. 05）. Conclusion The clinical effect of anus preserving operation for patients with low rectal cancer is obvious. The operation can improve the prognosis of patients and gain high satisfaction,and it is supposed to be widely promoted.
