组织多普勒超声评价地中海贫血胎儿的心脏纵向功能

摘　　要：目的:采用组织多普勒超声评价胎儿纵向心功能,对比分析正常胎儿与地中海贫血胎儿数值,希望对重型α-地中海贫血胎儿产前预测非侵袭性检测提供新方法及理论依据。方法:运用彩色组织多普勒速度成像图技术,对中孕期重型α-地中海贫血胎儿及的正常胎儿,左右心室壁及室间隔的9个观察点的速度曲线进行分析,测量收缩期运动速度Vs,进行统计学分析。结果:1对照组及重型α-地中海贫血胎儿左心室、右心室及室间隔的运动峰值速度均为基底部>中部>心尖部;2重型α-地中海贫血胎儿胎儿室壁运动速度明显小于正常对照组。结论:重型α-地中海贫血胎儿与正常胎儿的心脏纵向功能存在差异,组织多普勒技术可对中孕早期或中期评价出这些差异,为预测胎儿地中海贫血提供全新的无创性检测方法及理论依据。Objective This research adopts the measure of tissue Doppler to assess the fetal longitudinal heart function through a contrastive analysis of the data between normal and thalassemia fetus,aiming to provide new ways and theoretical basis for non- invasive antenatal diagnosis of fetus with severe α- thalassemia. Method This research utilized the color tissue Doppler velocity imaging technology to analyze the velocity curve consisted of data from nine observation points of the left and right ventricular wall and interventricular septum of both severe α- thalassemia fetus and normal fetus in their mid- gestation. The velocity of systole（ Vs） was measured and statistically analyzed. Results 1For control group and severe α- thalassemia fetus,the peak movement velocity of left and right ventricle and interventricular septum showed a trend of basilar part central section cardiac apex; 2The movement velocity of ventricular wall with severe α- thalassemia fetus was obviously less than the normal control group. Conclusion There exists difference between the heart longitudinal function of severe α- thalassemia fetus and normal fetus,and tissue Doppler technology can help diagnose the differences from mid or early gestation,thus providing theoretical basis and non- invasive method for prediction of thalassemia of fetus.
