盐酸达泊西汀治疗早泄的有效性及其对精液参数的影响

摘　　要：目的观察盐酸达泊西汀(必利劲)治疗早泄的有效性及其对男性生育能力的安全性。方法收集2014年2月至2015年2月以早泄就诊的门诊患者42例为志愿者,采用自身前后对照研究,试验观察10周,每个志愿者分别在入组前,口服必利劲4周和8周,以及停药2周后分别进行访视。参照早泄诊断问卷(主要疗效指标)和受试者及性伴侣早泄临床综合印象变化表(次要疗效指标)来评估必利劲治疗早泄的效果。通过比较治疗前后精液分析中精子总数、浓度、总活力、前向运动精子比率的变化,来评估必利劲对男性生育能力的影响。结果治疗前后,患者早泄诊断问卷(PEDT)得分明显降低,差异有统计学意义(P<0.05)。治疗后,患者及其性伴侣填写早泄临床综合印象变化表,得分明显提高,差异有统计学意义(P<0.05)。治疗前后患者精液分析中精子总数、精子浓度、精子总活力、前向运动精子比率及精子形态等主要指标的比较无明显变化,差异无统计学意义(P≥0.05)。结论必利劲30mg按需服用,能有效治疗早泄,且短期对青年男性精液质量无明显影响。Objective To observe the efficacy and safety of Dapoxetine in the treatment of premature ejaculation. Methods From February 2014 to February 2015, 42 patients with premature ejaculation were recruited in the study, and self-controlled method was used to observe 10 weeks. In 4 weeks, 8 weeks after usage of Dapoxetine, and 2 weeks after drug withdrawal, premature ejaculation indicators of the patients were assessed. Premature ejaculation diagnostic questionnaire （primary index） and the clinical global impression of subjects and sexual partners premature ejaculation table changes（ the secondary efficacy index） were applied to evaluate the clinical effect of Dapoxetine in the treatment of premature ejaculation. By comparing the changes before and after treatment, the total number of sperm in semen analysis, the total activity concentration, motile, will be to assess the effect of Dapoxetine on the male fertility. Results After treatment, the PEDT score of patients with premature ejaculation （PEDT questionnaire） was significantly decreased, the difference was statistically significant （P〈0.05）, and the score of clinical comprehensive impression change table in premature ejaculation patients and their sexual partners was significantly increased, the difference was statistically significant （P〈0.05）. Meanwhile, the total number of sperm in a semen analysis, sperm concentration, motility, the former to the sperm motility rate of sperm morphology and other major indicators had no significant difference after treatment （P〉0.05）. Conclusion Dapoxetine with a dose of 30mg is effective for treatment of premature ejaculation and has no significant effect on the quality of semen.
