中西医联合治疗慢性乙型肝炎肝纤维化的临床观察

摘　　要：目的探讨中医小柴胡汤联合西医阿德福韦酯治疗乙肝肝纤维化的临床疗效。方法选取我院2013年5月~2015年5月收治的乙肝肝纤维化患者共81例,按照入院时间顺序随机分为对照组（n=40）与观察组（n=41）,对照组患者接受阿德福韦酯治疗,观察组接受阿德福韦酯联合小柴胡汤治疗。比较两组患者肝功能、肝纤维化指标、病毒转阴率。结果治疗后观察组肝功能指标ALT、AST、ALB及TIBL均明显优于对照组,（t=9.3475,7.1545,-3.6441,4.5932,P〈0.05）。观察组肝纤维化指标HA、LN、PV-Ⅲ、IV-C均低于对照组,（t=13.9750,6.7464,4.1195,5.0008,P〈0.05）。两组患者HBs Ag转阴率及HBe Ag转阴率差异无统计学意义（P〉0.05）。观察组HBV-DNA转阴率明显高于对照组（x^2=4.2703,P〈0.05）。结论小柴胡汤联合阿德福韦酯能有效改善肝功能,抑制肝纤维化,提高病毒载量转阴率,安全有效,值得临床推广使用。Objective To explore clinical curative effect of Minor Bupleurum Decoction combined with adefovir dipivoxil in the treatment of chronic hepatitis B hepatic fibrosis. Methods 81 patients with hepatitis B liver fibrosis who were admitted to our hospital from May 2014 to May 2015 were selected and randomly divided into the control group（n=40） and the observation group（n=41）.The control group was treated by adefovir dipivoxil while the observation group was treated by adefovir dipivoxil combined with Minor Bupleurum Decoction.Liver function indexes,liver fibrosis indexes and negative conversion rate of virus of patients in two groups were compared. Results After treatment, liver function indexes such as ALT,ALB,AST and TIBL of the observation group were all significantly better than those of the control group,t=9.3475,7.1545,-3.6441,4.5932,P 0.05.The liver fibrosis indexes such as HA, PV- Ⅲ,LN and IV-C in the observation group were all lower than those in the control group（t=13.9750,6.7464, 4.1195,5.0008,P〈0.05）.Negative conversion rates of HBs Ag and HBe Ag of two groups had no significantly statistical difference（P 〈0.05）.Negative conversion rate of HBV-DNA of the observation group was significantly higher than that of the control group（x^2=4.2703,P 0.05）. Conclusion Minor Bupleurum Decoction combined with adefovir dipivoxil can effectively improve liver function, inhibit liver fibrosis and increased load negative conversion rate of viruses.It is safe and effective, worthy of clinical promotion and application.
