三种根管封闭剂根管充填术后疼痛的临床观察

摘　　要：目的:观察Cortisomol、AH-Plus、Apexit-Plus根管封闭剂在根管充填术后2周内疼痛随时间变化的关系,为根管治疗术疼痛的管理提供参考数据。方法:选择需要做根管治疗的患牙324颗,根管预备后封入氢氧化钙2周后来诊,随机分为A、B、C三组,分别以Cortisomol、AH-Plus、Apexit-Plus根管封闭剂配合标准牙胶尖用冷牙胶侧方加压法完成根管充填,术后疼痛标准参照Negm分为四级,实验数据由SPSS 22.0软件分析完成,使用卡方列联表检验比较三组患者不同疼痛等级的差异。结果:三组患者在术后无4级疼痛发生,2、3级疼痛在术后即刻达峰值,3级疼痛3h内迅速减少,6h后趋于缓解;2级疼痛1d内迅速减少,2d后下降减缓,2周仍有发生;1级疼痛1d内快速增加,2d后放缓;三组患者在不同时间点各疼痛等级及数量上比较无显著性差异(F4=2.071,P=0.7226)。结论:三组患者根管充填后即刻疼痛的数量和等级均达峰值,完全无疼痛的患者随时间增加快速增加,2周后仍有2级疼痛的患者,不同时间点三组患者疼痛数量及不同等级的疼痛无显著性差异,从短时间术后疼痛考虑三种根管封闭剂均是临床值得推荐的材料。Objective To observe and compare the effect of three different root canal sealers including Cortisomol, AH-Plus and Apexit-Plus on patient pain level after root canal filling operations during two weeks, for therapeutic reference. Methods A total of 324 samples were chosen for the operation. All teeth were sealed with calcium hydroxide after root canal preparation for two weeks and randomly divided into three groups named A, B and C. Canal filling operations were done with cold teeth lateral compression technique method using Cortisomol, AH-Plus and Apexit-Plus respectively. Pain levels were ranked according to Negm＇s standard and divided into 4 levels. Statistics were done using IBM SPSS 22.0 with Chi-square and Fisher＇s exact test. Results No level 4th pain was found in all groups. Level 2 and level 3 reached peak soon after operation. Level 3 pain rapidly decreased after 3 hours and reached plateau after 6 hours. Level 2 pain dramatically decreased after 1 day and slowly declined after 2 days, but still happened in two weeks. Level 1 pain rapidly increased within 1 day and decreased after 2 days. No difference was observed among all three groups in terms of pain level and patient numbers（F4=2.071, P=0.7226）. Conclusions All three groups reached peak pain level soon after operation. No perceivable pain level was increased among patients rapidly after operation. Level 2 pain was still found after 2 weeks. Since no difference was observed for level 3 pain among all groups in terms of patient numbers, all three root canal sealers could be recommended when considering short-term pain levels.
