精神分裂症患者第2代抗精神病药治疗中主观感受及相关分析

摘　　要：目的:探讨首发精神分裂症住院患者第2代抗精神病药治疗中的主观舒适度及其与精神症状、病情严重程度、药物不良反应等的相关性。方法:收集首发精神分裂症住院患者共193例,随机选用抗精神病药(利培酮、奎硫平、奥氮平)进行治疗。在入组时、治疗4周和治疗8周采用阳性与阴性症状量表(PANSS)、临床总体印象量表(CGI)及治疗中出现的症状量表(TESS)评估。在治疗4周和8周,患者自评抗精神病药治疗中主观舒适度量表(SWN)评估。结果:治疗4周时仅有阴性症状量表评分与SWN评分呈负相关(P<0.05)。在治疗8周后,患者的PANSS总分、各分量表评分以及CGI评分均与SWN评分呈负相关(P均<0.05)。患者的神经系统不良反应和心血管系统不良反应发生率均与其SWN评分呈负相关(P均<0.05)。结论:抗精神病药对精神症状、特别是阳性症状的控制能改善患者的主观舒适度,而神经系统不良反应及心血管系统不良反应会降低患者的主观舒适度。Objective： To investigate first-episode schizophrenia patients ＇ subjective well-being under second generation antipsychotics,and its relationship with clinical symptoms and side effects. Method： One hundred and ninty three inpatients meeting inclusion criteria were enrolled in this study,and randomly selected using second generation antipsychotics： quetiapine,risperidone or olanzapine. At the beginning of the study and the end of week 4 and week 8,positive and negative syndrome scale（ PANSS）,clinical global impressions-severity of illness scale（ CGI） and treatment emergent symptoms scale（ TESS） were evaluated by psychiatrists.Subjective well-being under neuroleptic treatment scale（ SWN） was performed by patients themselves.Results： At the end of week 4,only negative scale scores had negative correlation with SWN（ P〈 0. 05）. At the end of week 8,PANSS total score,all subscales score and CGI score had negative correlation with SWN. Side effect of nervous system and cardiovascular system had negative correlation with SWN（ P 〈0. 05）.Conclusion： The effect of antipsychotics on psychiatric symptoms,especially the control of positive symptoms can improve patients＇ subjective well-being,while the side effect of nervous system and cardiovascular system can reduce the patients＇ subjective well-being.
