子宫内膜癌患者不同病期血小板参数变化及其临床意义

摘　　要：目的探讨子宫内膜癌（endometrial carcinoma,EC）患者血小板参数与临床分期的关系。方法收集我院2010-01～2015-11的153例初治EC患者的临床病理资料,并以139例活检结果为正常子宫内膜的妇科患者为对照组,探讨血小板计数（PLT）、平均血小板体积（MPV）、血小板分布宽度（PDW）和血小板比积（PCT）在EC中的水平变化及其与临床病理分期的相关性。结果 EC患者血清中PLT、MPV、PDW、PCT水平较对照组高,差异有统计学意义（P〈0.05）。EC中Ⅲ期及Ⅳ期患者血清中PLT、MPV、PDW、PCT水平均较Ⅰ期及Ⅱ期高,差异有统计学意义（P〈0.05）。存在盆腔淋巴结转移组血清中PLT、MPV、PDW、PCT水平较无淋巴结转移组高（P〈0.05）。子宫肌层浸润深度≥1/2组血清中PLT、MPV、PDW、PCT水平较浸润深度〈1/2组高（P〈0.05）。CA125增高组子宫内膜癌患者的血清中PLT、MPV、PDW、PCT水平较CA125水平正常组高,（P〈0.05）。EC患者术后血清中PLT、MPV、PDW、PCT水平比术前低（P〈0.05）。结论血小板数量增多及功能活化与子宫内膜癌的侵袭转移关系密切,血小板参数的改变可协助子宫内膜癌病情评估及预后判断。Objective To explore the association between platelet parameters and different stage of patients with endometrial carcinoma（ EC）. Methods The clinical data of 153 cases of newly diagnosed EC and 139 cases of normal endometrium（ control group） by pathological examination under colposcope from January 2010 to November 2015 in our hospital were analyzed retrospectively. The platelet count（ PLT）,mean platelet volume（ MPV）,platelet hematocrit（ PCT） and platelet distribution width（ PDW） were compared between EC patients and controls and between different clinicopathological stage of patients with EC. Results The levels of the platelet parameters were significantly higher in EC group than in control group（ P〈0. 05）. The levels of the platelet parameters were significantly higher in EC patients with FIGO stages Ⅲ and Ⅳ than in those with stagesⅠand Ⅱ（ P〈0. 05）,higher in patients with lymph node metastasis than in those without lymph node metastasis（ P〈0. 05）,higher in patients with muscle invasive depth≥1 /2 muscular layer than in those with muscle invasive depth 1 /2 muscular layer（ P〈0. 05）,higher in patients with CA125 35 U / ml than in those with CA125≤35 U / ml（ P〈0. 05）,but lower after operation than that before operation（ P〈0. 05）. Conclusion Increase of platelet count and its function activation are closely related to invasion and metastasis of EC. The platelet parameters may be helpful in disease assessment and prognosis evaluation in EC.
