微创术和锤凿术拔除老年人残根残冠的临床疗效分析

摘　　要：目的:探讨利用微创术和锤凿术拔除老年人残根残冠的临床疗效分析。方法:选取本院2013年5月至2015年5月收治的需行拔除残根残冠的符合研究条件的老年患者为研究对象,随机将患牙分为观察组140例以及对照组140例,对照组采用传统锤凿术拔除残根残冠,观察组采用牙科微创手术拔除残根残冠,分析两组患者手术时间、疼痛程度、拔牙窝完整性、断根发生率、干槽症及患者满意度情况。结果:观察组手术时间、疼痛程度、拔牙窝完整性评分均低于对照组,满意度评分高于对照组,差异有统计学意义(P<0.05),断根发生率、干槽症发生率观察组低于对照组,差异有统计学意义(P<0.05)。结论:对拔除老年人残根残冠患者应用微创技术治疗具有高效、安全、无痛的特点,值得临床应用推广。Objective： To investigate the clinical efficacy of minimally invasive surgery（MIS） and hammer chisel technique for extracting residual roots and crowns in the elderly. Methods：Elderly patients who needed to extract residual roots and crowns in our hospital from May 2013 to May 2015 were selected. They were randomly divided into experimental group（140 residual roots and crowns using MIS） and control group（140 residual roots and crowns using hammer chisel technique）. The operation time,postoperative pain,completeness score of extracted tooth,root fracture,dry socket and patient satisfaction scores were compared between these two groups. Results：The operation time,postoperative pain and completeness score of extracted tooth in patients treated with MIS were lower than that in those treated with hammer chisel technique,patients＇ satisfaction scores of the experimental group were higher than that of the control group, and there were significant difference among patients treated was different techniques（P〈0.05）. The incidence of tooth fracture and dry socket in patients treated with MIS were significantly lower than those in patients treated with traditional hammer and chisel？technique（P〈0.05）. Conclusions： Compared with hammer and chisel technique, MIS is an efficient, safe,painless method to extract residual roots and crowns of elderly patients, which is worthy of clinical application and promotion.
