生物联合手术治疗肾癌远期疗效的Meta分析

摘　　要：目的：系统评价生物联合手术治疗肾癌的远期疗效。方法：利用计算机检索万方数据库、维普期刊资源整合服务平台、中国知网期刊数据库、中国生物医学文献数据库,检索年限均为建库至2014年12月。按照Cochrane系统评价手册,对纳入文献的方法学质量进行评价,然后提取并整理数据,采用RevMan5.3统计软件进行Meta分析。结果：共纳入9个随机对照临床试验,包括1 281例患者。Meta分析结果显示：与单纯手术治疗组相比,生物治疗联合手术治疗能提高肾癌患者的3年生存率（78.91%vs 61.54%,OR=2.15,95%CI：1.57-2.93,P〈0.001）和5年生存率（82.20%vs 66.98%,OR=2.21,95%CI：1.55-3.13,P〈0.001）,但并不影响肾癌患者的1年生存率（97.18%vs 93.98%,OR=2.22,95%CI：0.93-5.27,P=0.07）。结论：生物治疗联合手术治疗能明显提高肾癌患者的远期疗效。Objective：To evaluate the long-term curative effect of biological therapy combined with operative treatment for patients with renal cell carcinoma.Method：The relevant randomized controlled trials were retrieved from the electronic databases of Wanfang Med Online,VIP,CNKI and SinoMed.The retrieved time limit was from the database construction to December 2014.The data were extracted from the eligible studies assessed for methodological quality according to Cochrane handbook for systematic reviews and analyzed by RevMan 5.3software.Result：Nine randomized controlled trials involving 1 281 patients were included for meta-analysis.The results indicated that,compared with operative treatment alone,the combination of biological therapy and operative treatment improved the three-year survival rate（78.91% vs 61.54%,OR=2.15,95%CI：1.57-2.93,P0.001）and the five-year survival rate（82.20% vs 66.98%,OR=2.21,95%CI：1.55-3.13,P0.001）for patients with renal cell carcinoma.However,there was no effect on the one-year survival rate（97.18% vs 93.98%,OR=2.22,95%CI：0.93-5.27,P=0.07）.Conclusion：The combination treatment of biological therapy and operation can obviously improve the long-term curative effect for patients with renal cell carcinoma.
