结直肠癌围手术期患者外周血T淋巴细胞CD69、HLA-DR表达变化及意义

摘　　要：目的观察结直肠癌患者围手术期外周血T淋巴细胞CD69、HLA-DR的表达变化,并探讨其临床意义。方法采用流式细胞仪检测40例结直肠癌患者(观察组)术前、术后第5、10天外周血T淋巴细胞CD69、HLA-DR的表达,并与同期检测15例健康体检者(对照组)进行比较,分析CD69、HLA-DR表达与临床病理参数的关系。结果观察组术前CD69、HLA-DR表达明显低于对照组(P<0.05),术后CD69、HLA-DR表达水平均较术前明显提高(P<0.05)。HLA-DR表达与淋巴结转移、远处转移、TNM分期有关(P<0.05),与其余临床病理参数无关(P均﹥0.05)。结论结直肠癌患者术前外周血T淋巴细胞CD69、HLA-DR表达明显降低,术后逐渐升高;检测二者表达有助于病情判断,进一步指导结直肠癌免疫治疗。Objective To observe the expression changes and significance of CD69 and HLA-DR in T lymphocytes of peripheral blood in perioperative period of patients with colorectal cancer. Methods Flow eytometry was used to detect the expression of CD69 and HLA-DR in T lymphocytes of peripheral blood in 40 cases of perioperative period（ including the preoperative and postoperative 5,10 days） of patients with colorectal cancer（ observation group）,and in 15 cases of healthy persons（ control group）. The relationship of their expression with clinicopathologic features was analyzed. Results The expression of CD69 and HLA-DR before surgery was significantly lower in the observation group than that of the control group（ P〈0. 05）,with further increase after surgery（ P〈0. 05）. The expression of HLA-DR was related with lymph node metastasis,distant metastasis and TNM stage after surgery（ P〈0. 05）,but was not related with the rest of clinicopathologic features（ all P〉0. 05）. Conclusions The expression of CD69 and HLA-DR before surgery in T lymphocytes of peripheral blood in the perioperative period of patients with colorectal cancer was significantly decreased,and gradually increased after surgery. The detection of CD69 and HLA-DR contributes to determining the state of an illness and guiding immune therapy for colorectal cancer.
