强脉冲光联合他克莫司软膏治疗面部激素依赖性皮炎疗效观察

摘　　要：目的探讨强脉冲光联合他克莫司软膏治疗面部激素依赖性皮炎的疗效及其安全性。方法选择我科门诊面部激素依赖性皮炎患者44例，外用0．1％他克莫司软膏1次／d，辅以地氯雷他定5mg、1次／d口服，连用4周；停用药物治疗后2周，采用强脉冲光治疗，每4周一次，共4次。于治疗后第4周（外用药治疗结束）和第22周（强脉冲光治疗结束后4周）随访观察，进行疗效评价，记录症状评分，计算疗效指数，记录局部不良反应等。结果44例患者中37例完成全部临床观察，第4周和第22周的有效率分别为62．2％和89．2％。结论强脉冲光联合他克莫司软膏治疗面部激素依赖性皮炎疗效确切，安全性好，是一种值得推广的治疗方法。Objective To investigate the efficacy and safety of intense pulsed light combined with tacrolimus oint- ment in treatment of facial hormone dependent dermatitis （HDD）. Methods A total of 44 HDD patients in our hospital were orally administrated with topical tacrolimus ointment once/day and 5 mg loratadine once/day, for four consecutive weeks. Two weeks after discontimtation, they were subjected to intense pulsed light treatment, once per four weeks, with a total of four times. On Week 4 （when topical treatment was finished） and Week 22 （four weeks after intense pulsed light treatment）, all patients were followed - up to evaluate efficacy. Their symptoms were scored to calculate correspond- ing efficacy index, while topical adverse reactions were recorded. Results Among 44 patients, 37 completed the whole clinical observation. The effective rate was 62.2% on Week 4 and 89.2% on Week 22. Conclusion Intense pulsed light combined with tacrolimus ointment can be applied in treatment of facial hormone dependent dermatitis, which is an effective and safe method and worth of widely clinical practice.
