小剂量洋地黄制剂在肺癌患者全肺切除术后的临床应用

摘　　要：目的探讨全肺切除术后应用洋地黄制剂的临床意义。方法回顾性分析2010年8月至2013年8月第四军医大学唐都医院胸外科78例肺癌全肺切除患者的临床资料。将患者分为两组:对照组39例,其中男27例、女12例,年龄(56.8±14.8)岁;试验组39例,其中男24例、女15例,年龄(57.4±10.1)岁。行单侧全肺切除术后,试验组给予口服小剂量洋地黄制剂,在第3、5、7天比较两组患者的动脉血氧分压(PaO_2)、肺动脉收缩压(SPAP)、平均动脉压(MAP)、肺动脉舒张压(DPAP)以及并发症的发生率。结果应用洋地黄制剂治疗后,试验组SPAP、MAP、DPAP低于对照组,差异有统计学意义(P<0.05)。与对照组比较,试验组心律失常发生率明显降低,差异有统计学意义(P<0.05);两组PaO_2、肺感染以及循环障碍无明显变化,差异无统计学意义(P>0.05)。结论小剂量洋地黄制剂可以改善单侧全肺切除术后患者的心功能。Objective To investigate the clinical significance of applying digitalis preparations after pneumonectomy.Methods We retrospectively analyzed the clinical data of 78 patients who underwent pneumonectomy in the Tangdu Hospital of The Fourth Military Medical University from August 2010 to August 2013.The patients were divided into a control group（39 patients with 27 males and 12 females at a mean age of 56.8±14.8 years） and a trial group（39 patients with 24 males and 15 females at a mean age of 57.4±10.1 years）.After pneumonectomy,the trial group received low dose of digitalis treatment.On 3,5,and 7 days,the arterial partial pressure of oxygen（PaO_2）,systolic pulmonary artery pressure（SPAP）,mean arterial pressure（MAP）,diastolic pulmonary artery pressure（DPAP）,and the rate of complications were examined.Results After treatment with the low dose of digitalis,the SPAP,MAP,DPAP of the trial group were statistically lower than those of the control group（P〈0.05）.Incidence of arrhythmia in the trial group was statistically lower than that of the control group（P〈0.05）.There was no statistical difference between the two groups in the PaO_2,the incidence of pulmonary infection,and circulation disorder（P〈0.05）.Conclusion Low dose of digitalis preparations can improve cardiac function after pneumonectomy.
