西甲硅油在胶囊内镜检查中的应用研究

摘　　要：目的观察西甲硅油在胶囊内镜检查前肠道准备中的清洁和祛泡效果。方法将行胶囊内镜检查的278例患者随机分成研究组和对照组,均使用聚乙二醇电解质散2袋口服进行肠道准备,研究组于检查前1 h服用西甲硅油乳剂30m L。观察两组患者肠道准备清洁度、肠道气泡量、不良反应发生情况、胶囊内镜小肠通过时间、全小肠检查成功率等指标。结果研究组肠道气泡量明显少于对照组(P<0.01),肠道准备过程中的不良反应发生率明显低于对照组(P<0.01)。两组患者肠道准备清洁度、胶囊内镜检查中的不良反应发生率、胶囊内镜小肠通过时间及全小肠检查成功率比较差异无统计学意义(P>0.05)。结论西甲硅油能够有效地去除胶囊内镜检查前肠道准备过程中产生的气泡,提高胶囊内镜检查时肠道视野清晰度,且能减少患者肠道准备过程中不良反应的发生。Objective To observe the effect of simethicone in the bowel preparation cleanliness and the removal of intestinal foam before the capsule endoscopy. Methods 278 patients who underwent a capsule endoscopy in our department were randomly divided into the study group and the control group. All the patients used 2 bags of polyethylene glycol electrolyte powder for bowel preparation,and the patients in the study group took 30 m L of simethicone emulsion at one hour before the examination. The bowel preparation cleanliness,the amount of foam,the adverse reactions,the small bowel transit time of capsule endoscopy and the success rate of small bowel examination were respectively observed in the two groups. Results The amount of foam in the study group was significantly lower than that in the control group（ P〈0. 01）,and the incidence of adverse reactions in the course of bowel preparation was also significantly lower than that of the control group（ P〈0. 01）. There were no significant difference in the bowel preparation cleanliness,adverse reactions rate in the process of capsule endoscopy,the small bowel transit time of capsule endoscopy and the success rate of small bowel examination between the two groups（ P〉0. 05）. Conclusion The intestinal foam which was produced in the process of bowel preparation before capsule endoscopy can be effectively removed by simethicone,so the visual field of the intestine was highly improved. In addition,simethicone can also reduce the incidence of adverse reactions in patients during the process of bowel preparation.
