大龄腭咽闭合不全患者咽后壁瓣成形手术前后异常语音特点的研究

摘　　要：目的探讨大龄腭咽闭合不全患者咽后壁瓣成形术手术前后异常语音的特点。方法采集施行咽后壁瓣成形术患者46例,于术前及术后一个月分别采集患者音频资料进行主客观综合评价,并选取元音/i/、辅音的不送气塞音/b/、送气塞音/p/、送气擦音/s//x/及不送气塞擦音/j//z/、送气塞擦音/c//q/等靶音,同步输入CSL-4150B计算机语音声学系统进行频谱分析。结果患者手术前后在语图、共振峰及语音清晰度等方面存在一定程度的差异,具有统计学意义,但远不能满足患者语音清晰度达到优等85%以上水平(正常值为96%以上)的需求。结论咽后壁瓣成形术可以改善大龄腭裂患者的语音功能,但依旧存在不同程度的病理性语音,仍需进行语音治疗。Objective To study the abnormal acoustic features of older velopharyngeal insufficiency patients with cleft palate before and after posterior pharyngeal flap transplantation.Methods Audio data of 46 older patients with cleft palate was collected before and one month after the operation. Speech intelligibility was evaluated and spectral analysis was conducted by CSL-4150 B acoustic system with the following input data： vowel / i /,unaspirated plosive / b /,aspirated plosives / p /,aspirated fricatives / s / and / x /,unaspirated affricates / j / and / z /,and aspirated affricates / c / and / q /. Results There were statistically significant differences in the sonogram,formant,and speech intelligibility before and after operation. However,the sampled patients was not be considered to be highly intelligible（ 85%） as the normal value was above 96%. Conclusion Although speech function of older patients with cleft palate could be improved by posterior pharyngeal flap transplantation,pathological voice still exists and speech treatment is required in the future.
