摘  要:目的探讨术中超声引导及神经电生理监测对显微手术治疗儿童海绵状血管瘤继发癫痫的指导作用和价值。方法回顾性分析显微手术治疗的6例以癫痫为临床表现的儿童海绵状血管瘤的临床资料。术中采用超声引导及皮层脑电图(ECo G)监测,显微镜下进行海绵状血管瘤全切除及致痫灶切除。结果术后随访2年以上,癫痫无发作,无神经功能障碍。结论术中超声引导及ECo G监测可精确定位海绵状血管瘤病灶及指导致痫灶切除范围,起到了既指导手术入路、减少脑组织损伤,又提高海绵状血管瘤全切率及癫痫治愈率的作用,值得临床推广。Objective To investigate the clinical significance of intraoperative ultrasound localization and electrophysiological monitoring in microsurgical treatment of cavernous hemangioma follow ed by epilepsy. Methods Clinical data of 6 cases of cavernous hemangioma in children w ith secondary epilepsy treated by microsurgical operation w ere retrospectively analyzed. Under the guidance of ultrasound and intraoperative electrocorticogram( ECo G) monitoring,cavernous hemangioma and epileptogenic foci w ere completely resected under microscope. Results During the follow-up of more than 2 years,no seizure or nerve dysfunction occurred. Conclusion Intraoperative ultrasound guidance and ECo G monitoring can accurately locate the cavernous hemangioma lesions and guide the resection of epileptic foci. It helps to guide the operation approach,reduce brain tissue injury,and improve the rate of total resection of cavernous hemangioma and cure rate of epilepsy,w hich therefore is w orthy of clinical promotion.
