头面部皮肤恶性肿瘤手术切除及创面修复与重建的方法探讨

摘　　要：目的研究并分析头面部皮肤恶性肿瘤手术切除和创面修复以及重建的方法。方法选取本院2012年8月至2014年9月期间所收治的54例头面部皮肤恶性肿瘤患者实行回归行分析,手术后根据患者具体创面情选取（MATRIDERM）人工脱细胞异体真皮进进行移植修复12例;10例原位缝合;8例局部皮瓣;7例邻位皮瓣进行重建和修复。结果有1例MATRIDERM移植创面的边缘出现感染,在换药医治伤口Ⅱ期已愈合,其他植皮和皮瓣均已成活,且供区以及MATRI DERM移植创面均是Ⅰ期愈合。对54例患者均进行随访,时间为1~3年,其中只有1例患者复发,在对其手术扩大切除后采取皮瓣移位修复,在随访1年过程中没有再次复发;全部患者均存活良好,功能以及外形满意。结论对于头面部皮肤恶性肿瘤应尽早诊断,测定切除,及时修复以及避免复发、功能和外观效果良好是医治的重点所在。根据患者肿瘤切面大小和周围组织情况,尽可能最大程度兼顾其缺损区域功能和外形的重建和修复。Objective To study and analysis of facial skin malignant tumor resection and wound repair and reconstruction methods. Methods Our hospital in August 2012 to September 2014, 54 cases treated by the head of facial skin malignant tumor patients in regression analysis, according to the specific wound mood to select patients after the operation（MATRIDERM） artificial cell allograft dermis in transplant repair off 12 cases; 10 cases of orthotopic suture; 8 cases of local skin flap; 7 cases of ortho position skin flap for reconstruction and repair. Results The edges of the wound infection, 1 case of MATRIDERM transplantation in the treatment to heal the wound Ⅱ period has healed, other skin grafts and flaps have survived, and donor area and MATRI DERM transplantation are Ⅰ healing of the wound. In 54 patients were followed up, time is 1 ~ 3 years, only 1 patients relapse, flap translocation in the extensive surgical resection after repair, in the process of followup of 1 year no relapse again; All patients were alive, good function and appearance. Conclusion The head of facial skin malignant tumor should be early diagnosis, determination of resection, timely repair, function and appearance effect is good and avoiding recurrence is the key of cure. According to the size and the surrounding tissue in patients with neoplasm situation, as far as possible, the greatest degree function and appearance of the defect areas rebuilding and repair.
