摘 要:目的评估1,3-β-D-葡聚糖在非血液标本中对放射性肺部真菌感染的诊断价值。方法分析肺癌患者放疗后血液、气道分泌物、胸水中1,3-β-D-葡聚糖(1,3-beta-D-glucan,BG)表达差异。结果不同标本G试验检测结果对放射性肺部真菌感染患者诊断效能存在差异。血液标本的敏感性、准确性、特异性均最高;阳性预测值和阴性预测值最优为胸水标本。结论在G试验阳性判断阈值为≥10pg/mL的条件下,胸水和气道分泌物标本的G试验检测结果能够检测肺癌患者放疗后肺部真菌感染。Objective To evaluate the diagnostic value of 1,3-β-D-dextran in non-blood samples for radioactive pulmonary fungal infection. Methods The differences of the expression of 1,3-β-D- dextran( 1,3-beta-D-glucan,BG) in blood,airway secretions and pleural effusion of lung cancer patients after radiotherapy were analyzed. Results G test results of different specimens were different in the diagnostic efficacy of the patients with radioactive pulmonary fungal infection. Sensitivity,accuracy and specificity of blood samples were the highest. And the positive predictive value and negative predictive value were optimal for the pleural effusion. Conclusion In the positive judgement threshold of G test is ≥10pg / mL,G test results of pleural effusion and airway secretions could detect lung fungal infection in patients with lung cancer after radiotherapy.
