摘  要:目的研究能谱CT容积穿梭扫描技术在烟雾病患者搭桥术术前及术后血流动力学的变化及血管形态显示方面的价值。方法 15例行搭桥手术的烟雾病患者分别在术前及术后行能谱CT容积穿梭扫描,获取术前及术后的全脑灌注及4D-CTA图像。同时,通过ASPECTS评分对术前、术后血流动力学变化进行快速半定量的评估。资料对比采用配对t检验。结果术后15例患者的搭桥血管均在4D-CTA上得以显示,术侧术前与术后的ASPECTS评分、大脑中动脉皮层分布区的CBV、TTP以及术前拟手术侧与对侧TTP差异均具有统计学意义(P<0.05)。结论能谱CT容积穿梭扫描技术在烟雾病患者搭桥术术前及术后血流灌注及血管形态的评估中具有重要的临床应用价值。Objective To investigate the value of spectral computed tomography( CT) volume shuttle scanning technique in assessment of hemodynamic changes and vascular status before and after superficial temporal artery-middle cerebral artery( STAMCA) bypass in the moyamoya disease. Methods Spectral CT volume shuttle scanning technique was performed in 15 patients with moyamoya disease before and after surgery. Preoperative and postoperative whole-brain perfusion images and four dimensional CT angiography( 4D-CTA) images reconstructed from perfusion source data were obtained in all patients. In addition,Alberta Stroke Program Early CT Score( ASPECTS) was used to offer rapid semi-quantitative evaluation of the hemodynamic changes.Paired t test was applied to assess the difference of the CT perfusion values. Results The bypass arteries of 15 patients were all demonstrated by the four computed tomography angiography( 4D-CTA). The preoperative and postoperative values of the CBV,TTP,ASPECTS as well as the preoperative TTP on the potential surgical side and contralateral side were significantly different(P〈0. 05). Conclusion Spectral CT volume shuttle scanning technique has great values for the preoperative and postoperative assessment.
