超声诊断毛母质瘤临床价值

摘　　要：目的探讨超声诊断毛母质瘤的临床价值。方法回顾性分析40例经手术及病理证实为毛母质瘤(研究组)的超声图像资料,并将超声特征与40例其他皮肤及皮下来源的肿块(对照组)进行对照分析。结果研究组的平均发病年龄、发病部位、病灶最大径线、病灶生长方式(病灶长轴与身体长轴的一致性)与对照组比较,差异均具统计学意义(P<0.05)。研究组病灶内部回声(是否均质及钙化情况)、周边低回声晕及后方声影与对照组逐一比较,两组差异均具统计学意义(P<0.05)。研究组的病灶形态及边界是否清晰两项与对照组比较,差异无统计学差异(P>0.05)。研究组病灶内部或周缘血流分布与对照组比较,差异无统计学意义(P>0.05)。结论分布于头面部或颈部的皮下低回声病灶,其长轴与身体长轴一致,且内部伴有钙化、周边低回声晕及后方声影时,应首先考虑为毛母质瘤。Objective To study the sonographic features of the pilomatricomas for precise diagnosis. Methods Sonographic images and clinical data of pilomatricomas that was confirmed by pathology in 40 patients（ the study group） were analyzed retrospectively,and the data were compared with the other kinds of subcutaneous tumors in 40 patients（ the control group）. Results1） There were statistically differences between the two groups of the mean age,the lesions＇ location,the lesions＇ largest diameters,and the lesions＇ growth direction consistent with the long axis of the bodies respectively（ P〈0. 05）; 2） There were statistically significant differences between the two groups of the echotexture（ homogeneous or not,the internal scattered calcification）,hypoechoic rim,and posterior acoustic shadow（ P〈0. 05）; 3） There was no statistical differences between the two groups of the shapes and margins of the leisions（ P〈0. 05）; 4） There was no statistically significant difference between the lesions＇ vascularity（ P〈0. 05）. Conclusion Pilomatricomas can be diagnosed precisely according to the following sonographic features： subcutaneous hypoechoic masses located in head or face or neck,growth direction consisting with the long axis of the body,internal calcification,peripheral hypoechoic rim and posterior acoustic shadow.
