摘 要:青光眼作为世界上第一大不可逆的致盲眼病,以视神经萎缩和视野缺损为特征性表现。目前,控制眼压仍是治疗青光眼的唯一有效的手段,然而很多青光眼患者特别是正常眼压性青光眼,其眼压能够长期维持在较低水平,而视野缺损的放大却仍然持续进展,让我们不得不思考眼压之外的可能性病因。近年来,青光眼发病机制的探索已上升到中枢神经系统水平,众多学者认为青光眼可能不仅仅是单纯的累及视神经的眼部疾病,其病变还可能累及整个视觉通路。因此,一些新的检查手段被应用在研究青光眼的发病机制以及早期诊断中。其中,多模态磁共振成像技术以其能够在活体中无创地检测中枢神经系统结构及功能的改变,而成为研究青光眼病因和发病机制的新思路。在本文中,笔者就近年来国内外有关磁共振成像的检查手段,以及运用于检测青光眼中枢神经系统改变的研究现状做一总结。As the largest irreversible blindness cause disease around the world,glaucoma is symbolized by optic atrophy and visual field defect. So far,pressure reduction remains the only effective way to treat glaucoma,but the visual field defect of many patients especially those with normal tension glaucoma continued to progress,whererese their intra ocularpressure can be maintained at a low level for a long time.So we have to seek the possible cause for the disease other than the IOP. In recent years,exploring the pathogenesis of glaucoma has risen to the level of the central nervous system,many scholars believe that glaucoma not only simply involving the optic nerve of the eye disease,but also involving the entire visual pathway. Thus,some of the new inspection methods are applied to the studies about the pathogenesis of glaucoma and early diagnosis. The advantage of multimodal magnetic resonance imaging technology is that it can detect the structure and function of the central nervous system in vivo non-invasively. Therefore it provide a new idea to study the cause and pathogenesis of glaucoma. In this essay,we make a summary of research status in term of domestic and foreign inspection methods of magnetic resonance imaging used in detecting the change of central nervous system.
