牙周基础治疗对中、重度慢性牙周炎伴高血脂患者血脂水平的影响分析

摘　　要：目的探讨牙周基础治疗对中、重度慢性牙周炎伴高血脂患者血脂水平的影响。方法选取2012年1月至2015年4月本院治疗的80例中、重度慢性牙周炎伴高血脂患者为研究对象,所有患者均接受牙周基础治疗,比较患者治疗前和治疗后3个月牙周探诊深度(probing depth,PD)、探诊出血(bleeding onprobing,BOP)、附着水平(attachment level,AL)、出血指数(bleeding index,BI)以及菌斑指数(plaque index,PLI),并采集患者静脉血,检测并比较治疗前后血脂水平。结果牙周基础治疗3个月后,患者的PD、BI、AL、PLI、BOP均较治疗前显著下降(P<0.05),血清总胆固醇(TC)、三酰甘油(TG)水平均较治疗前显著下降(P<0.05),高密度脂蛋白(HDL)水平较治疗前显著升高(P<0.05),治疗前后低密度脂蛋白(LDL)水平比较差异无显著性(P>0.05)。牙周基础治疗后3个月,80例高血脂患者中38例血脂水平降至正常,其余患者血脂未恢复至正常水平,但均较治疗前明显改善。结论牙周基础治疗不仅可以改善中、重度慢性牙周炎伴高血脂患者的牙周状况,还可提高牙周治疗效果,减轻牙周感染和牙周炎症,有效改善患者血脂水平,值得临床推广应用。Objective To investigate the effect of periodontal basic therapy to lipid level of patients with moderate or severe chronic periodontitis and hyperlipidemia. Method 80 patients with moderate or severe chronic periodontitis and hyperlipidemia treated in our hospital from January 2012 to April 2015 were selected as the research subjects. All patients were given periodontal basic therapy. The probing depth（PD）, bleeding onprobing（BOP）, attachment level（AL）, bleeding index（BI） and plaque index（PLI） were compared before and after treatment for 3 months, and venous blood of patients were collected and detected, lipid levels were compared before and after treatment. Result Patiens were treated for 3 months by periodontal basic therapy, the PD, BI, AL, PLI, BOP were significantly reduced than before treatment（P 0.05）, the serum TC and TG levels were significantly decreased compared with before treatment（P 0.05）, the level of LDL was significantly higher than before treatment（P 0.05）, the level of LDL had no difference before and after treatment（P 0.05）. After 3 months of treatment, 38 patients with hyperlipidemia were reduced to normal in 80 cases, and the rest were not recovered to the normal level, but were significantly improved compared with before treatment. Conclusion For patients with moderate or severe chronic periodontitis and hyperlipidemia, using periodontal basic therapy not only can effectively improve periodontal status and periodontal treatment, reduce periodontal infection and periodontal disease, and also can improve blood lipid levels which is worthy of promotion and application.
