双歧三联活菌联合乳果糖治疗肝硬化肠源性内毒素血症的疗效及其对肝功能和凝血功能的影响

摘　　要：目的探讨双歧三联活菌联合乳果糖治疗肝硬化肠源性内毒素血症的临床疗效及其对患者肝功能和凝血功能的影响。方法 2014年2月—2015年2月湖北省黄冈市中心医院消化内科诊治肝硬化合并肠源性内毒素血症患者68例,采用随机单盲法分为观察组和对照组各34例。对照组患者单予常规综合治疗,观察组患者同时给予双歧三联活菌胶囊和乳果糖口服液,对比临床治疗效果及患者肝功能和凝血功能指标变化。结果观察组总有效率88.2%高于对照组的67.6%（χ^2=9.56,P〈0.05）。治疗后,2组患者ET、TNF-α、IL-1α、IL-6及ALT、AST、TBil均明显低于治疗前,且观察组与对照组比较差异均有统计学意义（P均〈0.05）。治疗后,2组凝血功能指标P TA明显高于治疗前,PT、APTT均明显低于治疗前,且观察组与对照组比较差异均有统计学意义（P均〈0.05）。结论双歧三联活菌联合乳果糖治疗肝硬化肠源性内毒素血症疗效显著,能够有效改善患者的肝功能和凝血功能,具有临床应用和推广价值。Objective To explore the clinical efficacy of Bifid Triple Viable combined with lactulose in the treatment of cirrhosis of the liver enterogenous endotoxin hematic disease and its influence on liver function and blood coagulation function.Methods 68 cases of patients with liver cirrhosis complicated by enterogenous endotoxin hematic disease from February2014 to February 2015 in the Department of Gastroenterology of Hubei Huanggang Province Central Hospital were randomly divided into observation group and control group,34 cases for each group.The control group received conventional treatment,while the observation group was treated with the combination of the bifid triple viable capsules and the lactulose oral liquid,the effect of clinical treatment and the changes of liver function and blood coagulation function indexes of the patients were compared.Results The total effective rate of the observation group was 88.2%,significantly higher than 67.6%of the control group（χ^2 =9.56,P〈0.05）.After treatment,ET,TNF-α,IL-1α,IL-6 and ALT,AST,TBil were significantly lower than before treatment in both groups,and differences between the observation group and control group were statistically significant（P〈0.05）.After treatment,the coagulation parameter PTA of two groups were significantly higher than before treatment,and PT,APTT were significantly lower than before treatment,and differences between the observation group and control group were statistically significant（P〈0.05）.Conclusion Bifid triple viable combined with lactulose has a significant effect on the treatment of cirrhosis of the liver enterogenous endotoxin hematic disease,which can improve,liver function and blood coagulation function and that is worthy of clinical application and promotional value.
