重复使用含呋喃唑酮的三联根除方案治疗Hp根治失败病例的研究

摘　　要：目的观察含有呋喃唑酮的三联根治方案对Hp根除失败病例的疗效。方法Hp根治失败病例25例,无青霉素过敏史,停药3个月以上,给予含呋喃唑酮的三联根治方案(埃索美拉唑20 mg,阿莫西林1.0 g,呋喃唑酮0.1 g,均2次/d,口服),疗程10 d。停药4周后复查^(13)C-UBT,阴性为根除成功;对于阳性患者,观察其^(13)C-UBT检测值,若较治疗前下降2/3以上,为明显下降。停药1个月后,继续予以上述方案治疗10 d,停药4周后复查^(13)C-UBT。结果25例初治失败者,经第1次含有呋喃唑酮的三联方案治疗10 d后,18例转阴,根除率为72%(18/25),7例根治失败者中,2例患者的^(13)C-UBT检测值较前无明显改变,5例^(13)C-UBT检测值较前下降2/3以上,对该5例患者停药1个月后继续予以上述同一方案治疗10 d,停药4周后复查^(13)C-UBT,其中有4例转阴,根除成功,1例仍阳性。HP的累计根除率为88%(22/25)。结论含有呋喃唑酮的三联方案对初治失败病例的根除率为88%,且对第1次治疗后13C呼气试验检测值明显下降的患者可以在间隔一段时间后重复使用同一含有呋喃唑酮的三联方案。Objective To observe the effects of furazolidone-containing triple regimen on Hp eradication failure cases.Methods Twenty-five cases who failed in the past eradication therapy without penicillin allergy were enrolled.Furazolidone-containing triple therapy was prescribed at 3 months after eradication failure.The regimen was as follows：esomeprazole 20 mg,amoxicillin 1.0 g and furazolidone 0.1 g,twice daily for 10 d as a course.^（13）C-Urea Breath Test was carried out to detect the HP infection.HP was considered to be eradicated successfully for those with negative results of ^（13）C-UBT after therapy.Those still with high ^（13）C-UBT value after treatment were failure cases,but those whose ^（13）C-UBT value decreased significantly to 1/3 of the value before eradication therapy would take the furazolidonecontaining triple regimen again after another one month for 10 d and then repeat the ^（13）C-UBT 4 weeks after the therapy.Results For 25 eradication failure cases enrolled,18 cases were eradicated successfully after the first course therapy of furazolidone-containing triple regimen.The eradication rate was 72%（18/25）.Among the 7 failure cases,^（13）C-UBT value in 2 cases showed no decrease compared with that before therapy,while the other 5 cases＇ 13 CUBT value decreased significantly but remained positive.The 5 patients accepted the furazolidone-containing triple regimen again,and 4 of them proved to be successfully eradicated after the second course of therapy.The Hp accumulative eradication rate of furazolidone-containing triple regimen was 88%（22/25）.Conclusion The Hp eradication rate of furazolidone-containing triple regimen in those who fail in eradication therapy is 88%,and the regimen can be repeated for those whose ^（13）C-UBT value decreases significantly after the first course of therapy,and it is effective in the following treatment after a period of time.
