626例感染科用药咨询的分析

摘　　要：目的:了解感染科用药咨询的特点,分享临床药师解决用药咨询的工作经验。方法:整理某院2013年感染科医护人员、患者或其家属向临床药师提出的用药咨询,从咨询者、咨询方式、咨询内容、解答方法等方面统计分析。结果:2013年感染科医护人员、患者或其家属共提出626例次用药咨询,其中医生占88.5%,以当面咨询当面解答为主,咨询内容包括用法用量(20.4%)、药品不良反应(15.5%)、药动学参数(8.3%)、药物相互作用(7.8%)、肝肾功能不全者剂量调整(7.3%)、药理作用(6.5%)、药品价格(5.3%)等,解答方法包括参考药品说明书、疾病诊疗指南共识及其他文献作答等。结论:医师在制定药物治疗方案时需要临床药师提供药品用法用量、药物相互作用、药动学参数及肝肾功不全者剂量调整信息,用药过程中关注药物不良反应的判断与处置,临床药师参照药品说明书、临床诊疗指南及其他文献提供准确的药学信息,积极参与个体化治疗方案的制定与调整。OBJECTIVE To learn about characteristics and disciplines of drug counseling for infectious diseases and share experiences of pharmacists for solving these consultations.METHODS Drug consultations proposed by medical and nursing staff in department infectious diseases,patients or their family to clinical pharmacists in2013 were summarized,and analyzed statistically from consultants,consulting methods,contents and solutions.RESULTS A total of 626 medication consultations were raised by medical staffs,patients and their families,88.5% came from doctors.Most consultations were conducted face to face,including dosage（20.4%）,adverse drug reactions（15.5%）,pharmacokinetic parameters（8.3%）,drug interactions（7.8%）,liver and kidney dysfunction dose adjustments（7.3%）,pharmacological effects（6.5%）,drug prices（5.3%）.Solutions were based on medicine specifications,disease treatment guidelines/consensus,and other documents.CONCLUSION Information about dosage,drug interactions,pharmacokinetic parameters,liver and kidney dysfunction dose adjustments proposed by pharmacist is needed to be included in treatment programs.Meanwhile,effective supervision provides positive comments to judgment and disposition of adverse drug reactions.Information from medicine specification,disease treatment guidelines/consensus,and other documents provides the basis of their accuracy and reasonableness.Treatment programs can be adjusted in timely according to patients＇ conditions through pharmacists＇ collaboration.
