中晚期癌症患者支持治疗需求及影响因素分析

摘　　要：目的探讨癌症患者支持治疗需求的影响因素。方法应用癌症患者支持性需求简明问卷（SCNS-SF34）中文版对2014年12月-2015年8月就诊于本院肿瘤科326例中晚期癌症患者进行问卷调查。共收回有效问卷321份。患者年龄22-86（56.48±11.65）岁,其中男性177例（55.1%）,女性144例（44.9%）。结果中晚期癌症患者支持性照顾需求各领域得分由高到低依次为卫生系统和信息需求（2.67±0.99）、生理和日常生活需求（2.63±1.12）、病人照顾和支持需求（2.51±1.03）、心理需求（2.34±1.03）和性需求（1.39±0.79）。在影响癌症患者支持治疗需求的因素中,性别影响生理和日常生活需求及卫生系统和信息需求;年龄影响心理需求和性需求;疼痛和KPS评分影响生理和日常生活需求及心理需求;确诊时长影响性需求;目前治疗种类影响病人照顾和支持需求及卫生系统和信息需求。结论中晚期癌症患者在卫生系统和信息、生理和日常生活、病人照顾和支持、心理及性方面的需求未得到满足,癌症患者的需求受性别、年龄、疼痛、KPS评分、确诊时长及目前治疗种类等因素影响,医务人员需根据患者的不同需求给予相应的支持。Objective To explore the supportive care needs and its influencing factors in cancer patients. Methods The 34-item short form Supportive Care Needs Survey（SCNS-SF34） was performed in 326 intermediate and advanced cancer patients who were hospitalized in our department from December 2014 to August 2015. A total of 321 effective questionnaires were taken back. Age of patients ranged from 22 to 86（56.48 ± 11.65） years old, 177 cases（55.1%） were men and 144 cases（44.9%） were women. Results The intermediate and advanced cancer patients supportive care needs of different dimension ranked from high to low as follows, health systems and information needs（2.67±0.99）, physical and daily life needs（2.63±1.12）, patient care and support needs（2.51±1.03）, psychological needs（2.34±1.03） and sexual needs（1.39±0.79）. Cancer patients physical and daily life needs and health systems and information needs were affected by gender, psychological and sexual needs by age, physical and daily life needs and psychological needs by pain and KPS score, sexual needs by final diagnosis time, and patients care and support needs and health systems and information needs by present types of treatment. Conclusion Intermediate and advanced cancer patients still have a great deal of unmet needs in health systems and information, physical and daily life, patient care and support, psychological needs and sexual domain. Gender, age, pain, KPS score, time of final diagnosis and present types of treatment are the influencing factors of cancer patients＇ needs. Patients should be supported by medical staff according to their different needs.
