摘  要:目的探讨早期雌激素替代治疗对特纳综合征(TS)患者骨密度(BMD)改善的效果。方法选取2005年8月到2015年8月就诊于北京协和医院内分泌科矮小门诊的27例TS患者,全部测量身高、体质量,雌激素治疗前后的骨密度由LUNAR-PRODGIY型骨密度仪进行测量。比较TS患者雌激素替代治疗前后骨密度的差异,治疗起始年龄对骨密度的影响。结果应用雌激素治疗后1年内、1~3年、>3年L2-4的骨密度分别为(0.96±0.13)g/cm^2、(0.92±0.13)g/cm^2、(0.93±0.14)g/cm^2,显著高于基线水平分别为(0.73±0.08)g/cm^2、(0.70±0.13)g/cm^2、(0.75±0.07)g/cm^2;治疗起始年龄<18岁组L2-4骨密度(0.96±0.14)g/cm2显著高于治疗起始年龄≥18岁组(0.90±0.10)g/cm^2。结论早期雌激素替代治疗可更有效地改善TS患者骨密度,但影响骨密度的其他因素仍需进一步深入探究。Objective To explore the effect of early estrogen replacement therapy on bone mineral density (BMD) in patients with Turner syndrome(TS). Methods 27 TS patients were selected from Nanism Clinic of Endocrinology Department of Peking Union Medical College Hospital from August 2005 to August 2015. Heights and weights were measured in all patients. The BMD was measm'ed by dual-energy X-ray absorptiometry (LUNAR-PRODGIY Type). We compared the BMD before and after estrogen replacement therapy in patients with TS and the effects on BMD of the starting age of estrogen treatment. Results Estrogen treatment after one year, 1 - 3 years, 〉 3 years, L2-4 BMD were (0. 96 ± 0. 13 ) g/cm^2, (0. 92 ± 0. 13 ) g/cm^2 and (0. 93 ± 0. 14 ) g/cruZ respectively, significantly higher than baseline (0.73 ± 0. 08 ) g/cm^2, (0.70 ± 0. 13 ) g/cm^2 and (0.75 ± 0. 07 ) g/cm^2. The BMD L2-4 of whose estrogen therapy was initiated 〈 18 years group (0.96 ± 0. 14 )g/cm^2 was significantly higher than the girls who started estrogen therapy ≥ 18 years group (0.90 ± 0. 10)g/cm2. Conclusions Early estrogen replacement therapy can effectively improve BMD in patients with TS,but other factors affecting BMD still need to be further explored.
