摘 要:目的通过比较干扰素、干扰素联合顺铂、顺铂在恶性胸腔积液治疗疗效方面的差异,探讨干扰素联合顺铂胸腔注药在恶性胸腔积液治疗中的应用价值。方法选取2011年3月至2014年12月期间就诊于我院呼吸科的恶性胸腔积液患者共144例,将以上患者随机分为三组,分别为干扰素组、干扰素联合顺铂组和顺铂组,每组患者均为48例,统计并计算各组中药物治疗的总体有效率,并比较各组在恶性胸腔积液治疗总体有效率方面的差异。结果干扰素组治疗总体有效率为62.5%,干扰素联合顺铂组的治疗总体有效率为85.4%,顺铂组的治疗总体有效率为66.7%,干扰素组和联合组间的治疗总体有效率存在统计学差异(χ2=6.54,P=0.01),顺铂组和联合组间的治疗总体有效率存在统计学差异(χ2=4.63,P=0.03),干扰素组和顺铂组间的治疗总体有效率之间无明显差异(χ2=0.18,P=0.67)。结论干扰素和顺铂对于恶性胸腔积液的治疗具有重要的意义,二者联合具有协同作用,可以显著提高恶性胸腔积液的治疗总体有效率,并且具有较高的安全性,值得临床推广及进一步研究。Objective: By comparing the differences between interferon,interferon + cisplatin and cisplatinin therapy of malignant pleural effusion,to explore interferon pleural injection combined with cisplatin application in the treatment of malignant pleural effusion. Methods: A total of 144 patients of pleural effusion treated in our hospital from March 2010 to December 2013 were enrolled in this study. The patients were randomly divided into three groups. Each group included 48 cases. Interferon group underwent chest interferon injection. Interferon + cisplatin group underwent interferon combined with cisplatin. Cisplatinin group underwent pleural cisplatin injection. The statistics for each group of drug therapy effectiveness were calculated and interferon,interferon + cisplatin and cisplatin were compared. There were differences in the efficiency of the treatment of malignant pleural effusion. Results: The overall effective rate of interferon treatment group was 62. 5%,that of the treatment of interferon in combination with cisplatin was 85. 4%,and the overall treatment of cisplatin effective rate of 66. 7%. The overall efficiency of treatment of interferon group and the combination group had significant difference( χ^2= 6. 54,P = 0. 01). There was significant difference in the overall efficiency between cisplatin treatment group and the combination group( χ^2= 4. 63,P = 0. 03). There was no significant difference between the overall efficiency of treatment between the interferon group and cisplatin group( χ^2= 0. 18,P = 0. 67). Conclusion: Interferon and cisplatin have important implications for the treatment of malignant pleural effusion,and their combination can significantly improve the treatment of malignant pleural effusion overall efficiency,which has high security worthy of promotion.
