多奈哌齐治疗合并高同型半胱氨酸血症早期阿尔茨海默病的临床观察

摘　　要：目的探讨多奈哌齐治疗合并有高同型半胱氨酸血症早期阿尔茨海默病的临床价值。方法选择2012年1月至2015年3月我院收治的80例合并有高同型半胱氨酸血症的阿尔茨海默病患者按照随机数字法分为两组,每组各40例,对照组口服叶酸、维生素B12片、盐酸美金刚片进行治疗,研究组在对照组基础上加用多奈哌齐,随访3个月,比较两组治疗前后MMSE及ADL评分变化情况、治疗过程中发生的药物不良反应及治疗前后的同型半胱氨酸浓度。结果治疗后研究组MMSE评分及ADL评分显著高于对照组,差异有统计学意义(P<0.05),治疗过程中,两组发生失眠、头晕、恶心、腹泻及尿失禁等不良反应的发生率方面无统计学意义(P>0.05),治疗后研究组同型半胱氨酸浓度显著低于对照组(P<0.05)。结论应用多奈哌齐可以提高合并有高同型半胱氨酸血症早期阿尔茨海默病的疗效,降低血清中同型半胱氨酸含量,促进患者认知功能及日常生活能力的改善。Objective To investigate the clinical effect of Donepezil in the treatment of for early alzheimer＇ s disease complicated with high homocysteine. Methods 80 cases of early alzheimer＇ s disease complicated with high homocysteine who got medical treatment in our hospital form January 2012 to March 2015 were randomly divided into two groups, each group had 40 patients, patients in control group were treated with oral folic acid, vitamin B^2 tablets and memantine hydrochloride tablets, while those in research group were added with Donepezil on the basis of control group, all of those patients were followed-up with 3 months, then the MMSE and ADL score and adverse reactions during the course of the treatment were compared. Results After treatment, the MMSE score and ADL score of research group were higher than that of control group （P〈0.05）, the differences reached statistical level, the proportion of insomnia, dizziness, nausea, diarrhea and uroclepsia of the two groups had non- statistically differences （P〉 0.05 ）, after treatment, the homocysteine concentrations of research group was considerably lower than control group （P 〈 0.05 ）. Conclusion The application of Donepezil in the treatment of early alzheimer＇ s disease complicated with high homo- cysteine would contribute to enhancing the clinical effect of the treatment, reducing serum homocysteine contents and improving patients＇ cognitive function and activities of daily living.
