摘  要:目的将血管内皮生长因子(vascular endothelial growth factor,VEGF)与体外培养的人脐带间充质干细胞(human umbilical cord derived mesenchymal stem cells,hUCMSCs)联合植入脑缺血(middle cerebral artery occlusion,MCAO)模型大鼠脑内,观察神经功能改善情况。方法选择75只SD大鼠,实验分组如下:MCAO组,仅制备缺血模型;假手术组,仅分离颈总动脉,不栓塞;VEGF组,缺血24 h后,局部缺血区给予VEGF治疗;hUCMSCs组,缺血24 h后,局部缺血区给予hUCMSCs治疗;hUCMSCs+VEGF联合组,缺血24 h后,局部缺血区给予hUCMSCs+VEGF治疗。观察缺血前、缺血后1 d、4 d、7 d、14 d、21 d各个时间点改良神经功能评分(mNSS)变化。结果hUCMSCs的增殖及分化不受Brdu标记的影响;缺血后,随着治疗时间的延续,单因素治疗组内(VEGF组、hUCMSCs组)大鼠的mNSS评分减低,联合组治疗效果优于单独因素处理组,且在不同时间点之间有显著性差异(F=5.018,P<0.01)。MCAO组的mNSS评分高于VEGF组、hUCMSCs组和联合组,差异有统计学意义(P<0.05),在所选择观察的6个时间点内,联合组21 d时mNSS评分得分最低。结论 VEGF联合hUCMSCs移植入MCAO模型大鼠脑内,可促进缺血模型大鼠的神经功能恢复。Objective To implant vascular endothelial growth factor (VEGF) and human umbilical cord derived mes- enchymal stem cells (hUCMSCs) together into the brain of model rats with middle cerebral artery occlusion (MCAO) and observe the nerve function improvement situation. Methods Seventy-five SD rates were divided into the following groups: MCAO group, in which only isehemia model was established, fake surgery group, in which only common carotid artery was separated without embolism, VEGF group, in which VEGF treatment was given in local ischemic region 24 hours after ischemia, hUCMSCs group, in which hUCMSCs treatment was given in local ischemic region 24 hours after ischemia, and hUCMSCs+VEGF combination group, in which hUCMSCs+VEGF treatment was given in local ischemic region 24 hours after ischemia. The changes of modified neurological severity scores(mNSS) before ischemia and 1 day, 4 days, 7 days, 14 days and 21 days after ischemia were observed. Results Proliferation and differentiation of hUCMSCs were not influenced by Brdu mark. After ischemia, as treatment continued, mNSS scores of the single factor treatment groups (VEGF group and hUCMSCs group) reduced, and combined treatment group was better than single fac- tor treatment group in treatment effects, with significant differences at different time points(F=5.018, P〈0.01). Compared with the VEGF group, hUCMSCs group and combined group, the score of mNSS of the MCAO group was higher, and the difference was statistically significant (P〈0.05), score of mNSS in the combined group showed the lowest scoring at the 21 d, in the selected six time point. Conclusion Combined transplantation of VEGF and hUCMSCs into brain of MCAO model rates can promote the nerve function recovery of ischemic model rats.
