重组人白介素-11治疗鼻咽癌放化疗所致急性口腔黏膜炎疗效观察

摘　　要：目的观察重组人白介素-11（IL-11）治疗鼻咽癌放化疗所致急性口腔黏膜炎的疗效。方法将64例在治疗过程中出现2级及以上的急性口腔黏膜炎的鼻咽癌患者随机分为观察组和对照组各32例；对照组以地塞米松、利多卡因及庆大霉素溶液漱口方法治疗，观察组在此基础上加用重组人白介素-11治疗。治疗2周后行疗效评价．同时观察口腔疼痛缓解情况及两组生活质量改善情况。结果观察组的急性口腔黏膜炎有效率为90．63％，对照组为68．75％，两组比较差异有统计学意义（χ^2=4．73，P=0．03）；观察组口腔疼痛缓解的有效率为87．50％，对照组为59．38％，差异具有统计学意义（χ^2=6．488，P=0．022）；观察组Karnofsky评分提高率为59．38％，明显高于对照组的31．25％（χ^2=5．107，P=0．044）。结论联合重组人白介素-11治疗鼻咽癌放化疗所致口腔黏膜炎疗效更好，能有效减少口腔疼痛和改善生活质量，且使用方便，值得临床推广。Objective To observe the efficacy of recombinant human interleukin-11 （IL-11） in treatment of acute oral mucositis related with nasopharyngeal carcinoma chemoradiotherapy. Methods 64 patients with level 2 and above acute oral mucositis were divided into observation group （n=32） and control group （n=32） at random. Control group was treated by gargles with dexamethasone, lidocaine and gentamicin, and combined use of recombinant human IL-11 for observation group. The therapeutic effects were evaluated after two weeks. At the same time, the oral pain relief effect and the improvement of quality of life were evaluated. Results The total effective rate of acute oral mucositis in observation group was 90.63%, and the control group was 68.75%. The data of the two groups had significant difference （χ^2=4.73, P=0.03）. The effective rate of oral pain relief in observation group was 87.50% and 59.38% in the control group, the two groups had statistical differences （χ^2=6A88, P=0.022）. And there was significant differences in the improvement of Karnofsky as well （59.38% vs 31.25%,χ^2=5.107, P=0.044）. Conclusion Combined use of recombinant human IL-11 can achieve better therapeutic effects in treatment of acute oral mucositis caused by concurrent radiochemotherapy. At the same time it can effectively reduce oral pain and improve the quality of life. It is convenient to use, and this method is worth popularizing in clinical practice
