经肛门显微内镜治疗直肠肿瘤的临床观察

摘　　要：目的:探讨经肛门显微内镜治疗直肠肿瘤的临床疗效及安全性。方法:选取2013年4月-2015年4月本院收治的直肠肿瘤患者79例作为研究对象,按照手术方式不同分为对照组39例和观察组40例,对照组接受腹腔镜Dixon术治疗,观察组接受经肛门显微内镜手术治疗。比较两组的手术持续时间、术中出血量、术后排便时间、住院时间、术后近期并发症与远期结局。结果:观察组手术持续时间、术中出血量、术后排便时间及住院时间均明显低于对照组,比较差异均有统计学意义(P<0.05);观察组术后近期并发症发生率为7.5%,明显低于对照组的25.6%,比较差异有统计学意义(P<0.05);两组术后半年随访直肠癌复发率与转移率比较差异均无统计学意义(P>0.05)。结论:经肛门显微内镜术治疗直肠肿瘤具有较好的临床疗效,创伤小且安全性高,手术操作简便,值得临床推广。Objective：To investigate the clinical efficacy and safety in the treatment of rectal cancer by anus micro endoscope.Method：A total of 79 patients with rectal tumor were selected from April 2013 to April 2015 in our hospital as the research objects,they were divided into the control group of 39 cases and the observation group of 40 cases according to the different operation methods.The control group was given laparoscopic Dixon treatment and the observation group was given endoscopic microsurgery treatment.The duration of operation,amount of bleeding,postoperation defecation time,hospitalization time,the recent incidence rate of postoperative complications and long-term outcome of two groups were compared.Result：The duration of operation,amount of bleeding,defecation time and hospitalization time in the observation group were significantly lower than the control group,the differences were statistically significant（P〈0.05）.The incidence rate of postoperative complications in the observation group was 7.5%,which was significantly lower than 25.6% of the control group,the difference was statistically significant（P〈0.05）.There was no significant difference in the rectal cancer recurrence rate and metastasis rate of two groups after six months follow-up（P〉0.05）.Conclusion：The Anus Micro Endoscope has good clinical curative effect,small trauma,high safety and easy to operate,which is worthy of clinical promotion.
