摘 要:为了便于医生在肝癌微波消融手术时进行术前手术规划,本文开发了一套基于CT影像的术前微波消融治疗计划系统。采用基于VTK的三维可视化技术重建三维模型,精确分割肿瘤、血管和重要组织器官;根据肿瘤特征和微波消融损伤场,确定手术路径及微波消融设备的功率和时间。系统消融体积重建模型是基于离体猪肝实验建立的有效微波消融体积模型。此软件具有较好的三维显示效果,医生可以在微波消融术前根据重建出肿瘤的大小、点数和模拟微波消融效果,确定合适的微波发射功率和作用时间,从而有助于优化微波消融的手术治疗计划和进行实时消融治疗的疗效评估。Surgery planning in the tumor treatment is a core problem in microwave thermal ablation. The software for visualizing medical image was designed based on Visualization Toolkit (VTK), which could display the tumor, vessels and vital organs. Based on the two-dimensional cross-sectional images of tumor area outline, we analogized the process of microwave needle inserted into the tumor. The software could calculate the tumor area and the calculation of tissue temperature field using the finite element simulation. Preoperative surgery planning is performed by the surgeon in an interactively, including the pose of the microwave probe and the microwave working time and power. Our laboratory established a mathematical model, which was efficacy microwave ablation volume visualization model based on in vitro porcine liver experiments. According to the mathematical model, the Visualization Toolkit software rebuilt ablation volume at different parameters, such as microwavepower and function time. It is found that the software could be used to calculate the microwave ablation size of the tumor and visually display microwave ablation effect before the surgery. Moreover, it also could show better three-dimensional microware ablation tumor images in order to get the suitable microwave transmission power and function time. The result would help doctors to optimize plan microwave ablation treatment operation and real-time ablation efficacy evaluation.
