摘 要:目的探讨慢性丘脑梗死患者全脑灰质体积改变及与认知功能减退的关系。方法对26例慢性丘脑梗死患者(梗死组)和29名健康志愿者(对照组)进行神经心理学评价及MR扫描。分别计算全脑灰质、白质、丘脑梗死体积,并进行统计学分析。结果与对照组比较,梗死组简易精神状态检查量表(MMSE)评分、短延迟回忆、长延迟回忆、再确认及图形回忆评分均明显降低(P均<0.05);梗死组全脑灰质体积与对照组比较明显减小[梗死组:(774 291.49±42 370.85)mm^3,对照组(802 999.21±37 075.84)mm^3,t=2.442,P=0.018]。梗死组全脑白质体积与对照组比较,差异无统计学意义[(705 587.51±32 886.53)mm^3,(717 580.47±31 911.05)mm3,t=1.634,P=0.108]。梗死组全脑灰质体积与MMSE评分及图形回忆评分呈中度正相关(r=0.491、0.427,P=0.017、0.042);全脑灰质体积与短延迟回忆、长延迟回忆、再确认评分均无相关性(P均>0.05);丘脑梗死体积与全脑灰质体积、MMSE评分、短延迟回忆、长延迟回忆、再确认及图形回忆评分均无相关性(P均>0.05)。结论慢性丘脑梗死患者全脑灰质体积减小,可能是其认知功能减退的原因。Objective To explore the correlation between gray matter volume changes and cognitive impairment in chronic thalamus infarction patients. Methods Totally 26 patients with chronic thalamic infarction (infarction group) and 29 healthy volunteers (controlgroup) underwent neuropsychological test and MR scan. The volume of whole brain gray matter, whole brain white matter and thalamus infarction were calculated and statistical analysis was performed. Results Compared with control group, the scores of mini-mental state examination (MMSE), short-term memory, long-term memory, reconfirm and fig- ure recall all significantly decreased in infarction group (all P〈0.05) ; the whole brain gray matter volume significantly decreased in infarction group (['774 291.49±42 370. 851 mm3 vs [802 999.21±37 075.84]mm3 , t=2. 442, P=0. 018). There was no sig- nificant" difference in whole white matter volume in infarction group ([705 587.51 + 32 886.53]mm3 ) compared with that in control group ([717 580.47+31 911.05]mm3, t=1.634, P=0.108). There was a moderate positive association between whole brain gray matter volume and the scores of MMSE and figure recall (r=0. 491, 0. 427, P=0. 017, 0. 042), but no relationship between whole brain gray matter volume and the scores of short-term memory, long-term memory, reconfirm (all P〉0.05). There was no significant relationship between thalamus infarction volume and whole brain gray matter vol- ume, the scores of MMSE, short-term memory, long-term memory, reconfirm and figure recall (all P〉0.05). Conclusion The whole brain gray matter volume decreases in chronic thalamic infarction patients, which may be the reason for cogni- tive impairment.
