溃疡性结肠炎患者中西医维持治疗的对比研究及101例病例随访分析

摘　　要：[目的]比较中、西医维持治疗对溃疡性结肠炎（UC）复发率的影响并对101例患者的复发因素进行分析。[方法]在2000年1月~2013年12月于沈阳军区总医院明确诊断为UC的14~87岁的487例住院患者中,在自愿原则下,招募到慢性复发型及慢性持续型患者101例参与随访研究,在排除年龄及性别等差异性的前提下,将其分成中医组和西医组,分别给予中、西医维持治疗（中医组依据辨证论治原则辩证选方用药,西医组则行规范化治疗）,比较2种维持治疗期间的复发情况,并分析导致UC复发的因素。[结果]UC患者中、西医维持治疗的5年复发率差异有统计学意义（P〈0.05）,可以认为中医维持治疗效果优于西医维持治疗。通过对患者维持治疗期间的随访,发现慢性复发型与慢性持续型UC在饮食失宜、情志不遂、气候变化时更易发作。[结论]在辨证论治理论指导下,中医维持治疗在预防UC复发方面较西医维持治疗优势明显。饮食失宜、情志不遂、气候变化是复发的重要诱因。[Objective]To compare the influence of traditional Chinese medicine and western medicine maintenance treatment on recurrence rate of ulcerative colitis（UC）and analyze the factors related to the recurrence of 101 patients.[Methods]From January 2000 to December 2013,among 487 UC patients aging 14-87 in Chinese PLA General Hospital of Shenyang Military Area Command,101 cases of chronic relapsing and chronic persistent type were recruited under the principle of voluntary,and divided into Chinese medicine group and western medicine group.The former was based on the principles of syndrome differentiation and given individualized formula,and the latter underwent conventional standardized treatment.Recurrence rate of the two groups were compared and factors leading to the recurrence were analyzed.[Results]Recurrence rate after five years showed statistically significance difference between the two groups（P〈0.05）.Through the follow-up of the patients during maintenance treatment,UC of chronic relapsing and chronic persistent type were more likely to attack due to improper diet,emotional upset,climate change.[Conclusion]Under the guidance of theory of syndrome differentiation,Chinese medicine maintenance therapy in prevention of the UC recurrence had obvious advantages compared with western medicine.Improper diet,e-motional upset,climate change were the important causes of recurrence.
