口内入路联合穿颊器与口外入路治疗下颌角及升支骨折的对比研究

摘　　要：目的对比分析口内小切口联合穿颊器和传统口外入路内固定手术治疗下颌角及升支骨折的临床应用价值。方法采用前瞻性研究方法,将69例下颌角及升支骨折患者随机分为两组,观察组(n=35)采用口内小切口联合穿颊器手术进行治疗,对照组(n=34)采用传统口外入路手术进行治疗。对比两组的骨折愈合情况,并完成为期≥1年的随访,对比手术美观、面部神经及功能损伤情况。结果观察组的骨折愈合情况优于对照组(P<0.05)。两组患者的手术时间和术后随访时间无统计学差异(P>0.05),观察组美观度评分优于对照组,术后并发症发生率低于对照组,差异均有统计学意义(P<0.05)。结论口内小切口联合穿颊器治疗下颌角及升支骨折术源性损伤小、患者恢复快、并发症发生率低,有临床推广价值。Objective To analyze the clinical application value of intraoral incision combining transbuccal trochar instrumentation in the treatment of mandibular angle and ramus fractures by a compare study with extraoral incision surgery. Methods 69 patients with mandibular angle and ramus fractures were divided into 2 groups randomly in a prospective study. Patients in the observation group were treated with intraoral incision combining transbuccal trochar instrumentation surgery, and patients in the control group were treated with extraoral incision surgery. The fracture healing conditions were compared between groups. Further more, a follow-up（ ≥1year） were completed, and the facial aesthetic score, nerve and functional damage conditions were compared between the two groups. Results The fracture healing condition of the observation group was better than that of the control group（P〈0.05）. There was no significant difference between groups in surgery time and follow-up time（ P〈0.05）, but the facial aesthetic score of the observation group beautiful rating was higher than that of control group, and the postoperative complication rate was lower than that of the control group, the difference had statistical significance（ P〈 0.05）. Conclusion Intraoral incision combining transbuccal trochar instrumentation surgery has less damage to the patients, contributing to faster recover and lower complication rate, so, it has clinical popularization value.
