摘 要:目的:观察高压氧治疗小血管病变导致的后循环缺血性眩晕(PCIV)的临床疗效。方法:选取2014年8月—2015年2月在脑病科住院的80例经头磁共振成像(MRI)、磁共振血管造影(MRA)、磁共振扩散加权成像(DWI)确诊的后循环缺血性眩晕患者,随机分为高压氧治疗组40例和对照组40例,观察治疗前后及治疗7 d,14 d眩晕评估评分量表(DARS)评分、眩晕障碍调查表(DHI)评分及经颅多普勒超声(TCD)的变化。结果:高压氧治疗组与对照组比较,能明显改善眩晕患者的临床症状,两组患者治疗前后DARS评分和DHI评分比较,差异有统计学意义(P<0.01);两组患者TCD结果比较,治疗前高压氧治疗组异常率为82.5%(33/40),对照组异常率为85.0%(34/40);治疗后高压氧治疗组异常率为22.5%(9/40),对照组异常率为42.5%(17/40)。提示高压氧治疗和经典治疗均可改善椎基底动脉系统的血流动力学,但高压氧治疗组效果更为显著。结论:高压氧治疗能够快速、有效地改善小血管病变导致的后循环缺血性眩晕的症状。Objective: To observe the clinical efficacy of hyperbaric oxygen for posterior circulation ischemic vertigo( PCIV) caused by small vascular lesions. Methods: From August in 2014 to February in 2015,80 cases in hospital with with posterior circulation ischemic vertigo,diagnosed by magnetic resonance imaging( MRI),magnetic resonance angiography( MRA),diffusion weighted imaging( DWI),were randomly divided into hyperbaric oxygen treatment group( 40 cases) and control group( 40 cases). Assessment Rating Scale( DARS) score,vertigo disorder questionnaire( DHI) score and transcranial doppler ultrasonography( TCD) were observed before and after the treatment 7 days,14 days. Results: Hyperbaric oxygen therapy group compare to control group,can significantly improve the clinical symptoms in patients with vertigo,DARS and DHI scores of patients in two groups has obvious change before and after therapy( P〈 0. 01). Two groups of patients with TCD changes as a result,before expanding set of treatment the abnormal rate was 82. 22%( 37 /45),the control group before treatment the abnormal rate was 86. 67%( 39 /45),expansion treatment group after treatment the abnormal rate was 15. 67%( 7 /45),the control the abnormal rate was 37. 78%( 17 /45). Expansion and prompt treatment and classical treatment all can improve the hemodynamics of vertebral basilar artery system,but the effect of hyperbaric oxygen treatment group is more significant. Conclusion: Hyperbaric oxygen treatment can quickly and efficiently improve the small vascular lesions caused by posterior circulation ischemic symptoms of dizziness.
