摘　　要：目的分析56例Ⅰb2Ⅱa期宫颈癌新辅助化疗后病理形态改变及对病理诊断的影响。方法 56例Ⅰb2Ⅱa期宫颈癌患者分为2组。观察组采用新辅助化疗及子宫切除术,对照组采用子宫切除联合盆腔淋巴清扫术。比较2组患者的疗效、并发症、病理形态变化和诊断结果。结果观察组有效率为50.00%,显著高于对照组的33.33%(P<0.05)。2组并发症发生率无显著差异(P>0.05)。观察组中度和高度危险因素≥2的患者比例少于对照组,且中度危险因素为0的患者的比例也较高,差异有统计学意义(P<0.05)。观察组切缘残留阳性及淋巴结转移阳性显著高于对照组(P<0.05)。结论应用新辅助化疗后Ⅰb2Ⅱa期宫颈癌病理形态改变及对病理诊断效果显著。Objective To analyze the pathological changes of 56 cases ofⅠb2Ⅱa stage cervical cancer and its effect on the pathological diagnosis. Methods A total of 56 patients with Ⅰb2Ⅱa stage cervical cancer were divided into two groups. The observation group was treated with the new adjuvant chemotherapy and hysterectomy,while the control group was treated with hysterectomy combined with pelvic lymph node dissection. The efficacy,the complications of the treatment,the pathological changes and the diagnosis results were compared between two groups. Results The effective rate was 50% in the observation group,which was significantly higher than 33. 33% in the control group（ P〈 0. 05）. There was no significant difference in incidence rate of complications between two groups（ P〉 0. 05）. The ratio of patients with moderate and high risk factors（ more than or equal to 2） in the observation group were less than that of the control group,ratio of patients with moderate risk factors（ value was 0） was significantly higher than the control group,and there were significant differences（ P〈 0. 05）. The positive rates of surgical residual and lymph node metastasis in the observation group were significantly higher than those in the control group（ P〈 0. 05）. Conclusion Application of neo- adjuvant chemotherapy can significantly improve pathological changes and effect of pathological diagnosis in patients with Ⅰb2Ⅱa stage cervical cancer.
