原发性闭角型青光眼合并白内障超声乳化术后临床观察

摘　　要：目的 观察原发性闭角型青光眼（PACG）合并白内障行超声乳化吸除联合人工晶状体植入术疗效及眼前段结构变化.方法 临床病例对照研究.对2013年1月至2015年4月在奉贤区中医院眼科选取PACG合并白内障患者42例42只眼为青光眼组,行超声乳化术吸除联合人工晶状体植入术;选取同期白内障患者86例86只眼为白内障组.应用Lenstar 900光学生物测量仪测量比较两组术前前房深度、晶状体厚度、眼轴长度,术后3个月观察青光眼组视力、眼压情况及中央前房深度.结果青光眼组术后3个月视力均有不同程度的提高,视力0.1～0.4者16只眼占38.10％,0.5～0.8者21只眼占50％,≥1.0者5只眼占11.90％.术后眼压均有下降,控制在9.10～19.90 mmHg,与术前比较差异有统计学意义（P＜0.05）.术后中央前房深度有明显加深,与术前比较差异有统计学意义（P＜0.05）.青光眼组与白内障组比较,有较厚的晶状体,较短的眼轴,较浅的中央前房深度,差异有统计学意义（P＜0.05）.结论PACG合并白内障患者具有晶状体相对较厚、位置相对靠前,眼轴较短的解剖特点,行超声乳化术吸除联合人工晶状体植入术可以有效地降低眼压、提高视力.Objective To observe the primary angle-closure glaucoma （PACG） merge cataract phacoemulsification with intraocular lens implantation effect and changes in the anterior segment structures.Methods Forty-two patients （42 eyes） of PACG complicated with cataract were selected as glaucoma group,suction line phacoemulsification with intraocular lens implantation.Eighty-six patients （86 eyes） of cataract chosen at the same period as cataract group.Anterior chamber depth,lens thickness,axial length were measured and compared preoperative;and visual acuity,intraocular pressure （IOP） and anterior chamber depth of glaucoma group were observed three months after operation using Lenstar900 optical biometry.Results Visual acuity of glaucoma group improved in varying degrees three months after operation,vision from 0.1 to 0.4 in 16 eyes accounted for 38.10%,from 0.5 to 0.8 in 21 eyes accounted for 50%,≥ 1.0 in 5 eyes accounted for 11.90%.IOP was decreased statistically significant difference at 9.10-19.90mmHg compared with preoperative.Postoperative central anterior chamber depth deepened statistically significantly difference compared with the preoperative.Glaucoma group had a thick lens,shorter axial length compared with cataract group,and the shallower the central anterior chamber depth,the statistically significant differences.Conclusions PACG with cataract patients have relatively thick lens position relative to the front,shorter axial anatomical features,suction line phacoemulsification and intraocular lens implantation can effectively reduce intraocular pressure,improve vision.
