血清可溶性CD40配体和IL-18水平与脑梗死的关系分析

摘　　要：目的：研究血清可溶性CD40配体（sCD40L）和白细胞介素18（IL-18）水平与脑梗死的关系。方法：选择2012年5月~2014年12月于我院就诊的脑梗死患者70例纳入脑梗死组,同一时间段内在我院体检中心体检的70例健康志愿者纳入研究的健康组,分离血清并测定sCD40L、IL-18、细胞黏附分子1（ICAM-1）、血管细胞黏附分子1（VCAM-1）、CD11b、CD18、基质金属蛋白酶（MMP）10、解聚素-金属蛋白酶17（ADAM17）、ADAMTS12的含量,分离外周血单个核细胞并测定CD40、CD40L、NLRP3、凋亡相关点样蛋白（apoptotic speck-like protein containing a caspase recruitment domain,ASC）、Caspase-1的含量。结果：脑梗死组患者血清中sCD40L、IL-18、ICAM-1、VCAM-1、CD11b、CD18、MMP10、ADAM17、ADAMTS12的含量以及外周血中CD40、CD40L、NLRP3、ASC、Caspase-1的表达量显著高于健康组（P〈0.05）;脑梗死患者的神经功能缺损越严重,血清中sCD40L、IL-18的含量以及外周血中CD40、CD40L、NLRP3、ASC、Caspase-1的表达量越高（P〈0.05）;sCD40L、IL-18高水平组脑梗死患者血清中ICAM-1、VCAM-1、CD11b、CD18、MMP10、ADAM17、ADAMTS-12的含量显著高于低水平组（P〈0.05）。结论：血清可溶性CD40配体和白细胞介素18水平升高与脑梗死的发生以及病情严重程度相关。Objective：To study the relationship of serum soluble CD40ligand（sCD40L）and interleukin-18（IL-18）levels with cerebral infarction.Methods：A total of 70 cases of patients with cerebral infarction treated in our hospital from May2012 to December 2014 were selected as the cerebral infarction group of the research,70 cases of healthy volunteers receiving physical examination in Physical Examination Center of our hospital during the same period were included in the health group of the research,serum was separated to detect sCD40 L,IL-18,intercellular adhesion molecule（ICAM）-1,vascular cell adhesion molecule（VCAM）-1,CD11 b,CD18,matrix metalloproteinase（MMP）10,a disintegrin and metalloprotease（ADAM）17and ADAMTS12 contents,and peripheral blood mononuclear cells were separated to detect CD40,CD40 L,NLRP3,apoptotic speck-like protein containing a caspase recruitment domain（ASC）and Caspase-1contents.Results：sCD40L,IL-18,ICAM-1,VCAM-1,CD11 b,CD18,MMP10,ADAM17 and ADAMTS12contents in serum as well as CD40,CD40 L,NLRP3,ASC and Caspase-1expression levels in peripheral blood of cerebral infarction group were significantly higher than those of health group（P〈0.05）;the more severe the neurologic impairment in patients with cerebral infarction,the higher the sCD40 Land IL-18 contents in serum as well as the CD40,CD40 L,NLRP3,ASC and Caspase-1expression levels in peripheral blood（P〈0.05）;serum ICAM-1,VCAM-1,CD11 b,CD18,MMP10,ADAM17 and ADAMTS-12 contents in cerebral infarction patients with high levels of sCD40 Land IL-18 were significantly higher than those in low level group（P〈0.05）.Conclusions：Increased serum soluble CD40 ligand and interleukin-18 levels are related to the occurrence of cerebral infarction and disease severity.
