榄香烯注射液联合化疗用于胃癌疗效与安全性的Meta分析

摘　　要：目的：系统评价榄香烯注射液联合化疗用于胃癌的疗效与安全性,以为临床提供循证参考。方法：计算机检索中国生物医学文献数据库、中文科技期刊数据库、中国期刊全文数据库、万方数据库、Medline、EMBase、Cochrane图书馆、美国医师协会数据库,收集榄香烯注射液联合化疗用于胃癌（试验组）的随机对照试验（RCT）,对符合纳入标准的研究进行资料提取,并采用Cochrane系统评价员手册5.1.0进行质量评价后,采用Rev Man 5.3统计软件进行Meta分析。结果：共纳入7项RCT,合计434例患者。Meta分析结果显示,试验组患者的近期有效率[RR=1.38,95%CI（1.14,1.67）,P=0.001]、生存质量提高率[RR=1.36,95%CI（1.15,1.60）,P〈0.001]均显著高于单纯化疗组,试验组患者的Ⅲ~Ⅳ度白细胞抑制发生率[RR=0.58,95%CI（0.37,0.91）,P=0.02]、Ⅰ~Ⅳ度血红蛋白抑制发生率[RR=0.76,95%CI（0.60,0.95）,P=0.02]、Ⅰ~Ⅳ度血小板抑制发生率[RR=0.69,95%CI（0.52,0.91）,P〈0.001]、Ⅲ-Ⅳ度血小板抑制发生率[RR=0.53,95%CI（0.30,0.93）,P=0.03]、Ⅰ-Ⅳ度神经毒性反应发生率[RR=0.69,95%CI（0.52,0.93）,P=0.01]均低于单纯化疗组,差异均有统计学意义。结论：榄香烯注射液联合化疗用于胃癌的疗效与安全性均较好。OBJECTIVE：To systematically review the efficacy and safety of Elemene injection combined with chemotherapy in the treatment of gastric carcinoma,and provide evidence-based reference for clinical treatment. METHODS：Retrieved from CBM,CJFD,VIP Database,Wanfang Database,Medline,EMBase,Cochrane Library and American Medical Association Database,randomized clinical trials（RCT）about Elemene injection combined with chemotherapy in the treatment of gastric carcinoma were collected,Meta-analysis was performed by using Rev Man 5.3 software after data extracting and quality evaluating by Cochrane 5.1.0.RESULTS：Totally 7 RCTs were enrolled,involving 434 patients. Results of Meta-analysis showed recent efficacy [RR=1.38,95%CI（1.14,1.67）,P=0.001] and improvement rate of life quality [RR=1.36,95% CI（1.15,1.60）,P〈0.001] of Elemene injection combined with chemotherapy in the treatment of gastric carcinoma were significantly higher than only chemotherapy. The incidence of Ⅲ-Ⅳ leukemia inhibition [RR=0.58,95%CI（0.37,0.91）,P=0.02],incidence of Ⅰ-Ⅳ hemoglobin inhibition [RR=0.76,95%CI（0.60,0.95）,P=0.02],incidence of Ⅰ-Ⅳ platelet inhibition [RR=0.69,95%CI（0.52,0.91）,P〈0.001] and Ⅲ-Ⅳ[RR=0.53,95%CI（0.30,0.93）,P=0.03] and incidence of Ⅰ-Ⅳ neurotoxicity[RR=0.69,95%CI（0.52,0.93）,P=0.01] in test group were significantly lower than control group,the differences were statistically significant. CONCLUSIONS：Both efficacy and safety of Elemene injection combined with chemotherapy in the treatment of gastric carcinoma are good.
