HE4联合CA125对早期卵巢癌的诊断价值研究

摘　　要：目的探讨HE4联合CA125对早期卵巢癌的诊断价值。方法选取卵巢癌患者45例,盆腔良性包块37例和40例健康体检者。分别检测入选者血清HE4和CA125的浓度,对比分析二者对卵巢癌的诊断价值。结果 HE4和CA125在鉴别卵巢癌与非卵巢癌方面无显著差异,P>0.05;CA125在区分盆腔良性包块和健康体检者方面有显著差异,P<0.05;血清HE4检测卵巢癌的敏感度为86.7%(39/45),特异度为92.5%(74/80);血清CA125检测卵巢癌的敏感度为60.0%(27/45),特异度为85.0%(68/80),两种标志物检测卵巢癌的敏感度和特异度比较差异有统计学意义,P<0.05;HE4检测卵巢癌的ROC-AUC为0.884,CA125检测卵巢癌的ROC-AUC为0.809,联合HE4与CA125检测卵巢癌的ROC-AUC为0.906,Z检验结果显示,HE4与CA125相比差异无统计学意义,P>0.05;HE4与HE4-CA125相比差异无统计学意义,P>0.05;CA125与HE4-CA125相比差异有统计学意义,P<0.05。结论 HE4联合CA125检测早期卵巢癌价值优于HE4和CA125的单独检测,可作为临床鉴别诊断卵巢癌诊断的首选无创检测方法。Objective To investigate the value of HE4 combined with CA125 in the diagnosis of early ovarian cancer.Methods 45 patients with ovarian cancer,37 cases of pelvic benign masses and 40 cases of healthy patients were selected. The serum concentrations of CA125 and HE4 were detected,and the value of the 2 differential diagnosis for ovarian cancer were compared. Results There had no significant difference between HE4 and CA125 in the identification of ovarian cancer and non ovarian cancer,P〉0. 05. CA125 had statistically significant difference in differentiating pelvic benign masses and healthy persons,P〈0. 05. The sensitivity of serum HE4 to ovarian cancer was 86. 7%（ 39 /45）,specificity was 92. 5%（ 74 /80）,and the sensitivity of ovarian cancer was 60. 0%（ 27 /45） and specificity was 85. 0%（ 68 /80）. The sensitivity and specificity of the 2 markers for ovarian cancer were statistically significant,P〈0. 05. ROC-AUC of ovarian cancer detected by HE4 was 0. 884,CA125 detected ovarian cancer ROC-AUC was 0. 809,and HE4 combined with CA125 detection of ROC-AUC was 0. 906. Z test showed that there was no significant difference between HE4 and CA125,P〉0. 05. There was no significant difference between HE4 and HE4 combined with CA125,P〉0. 05,CA125 and HE4 combined with CA125 had statistically significant difference,P〈0. 05. Conclusion The value of HE4 combined with CA125 is better than that of HE4 and CA125 alone. It can be used as the diagnosis method in clinical differential diagnosis of ovarian cancer.
