阴道镜与宫颈HPV检测对宫颈癌及癌前病变检出率的临床对比研究

摘　　要：目的对比分析阴道镜与宫颈HPV检测对宫颈癌及癌前病变的检出率。方法选取行宫颈病变筛查的540例已婚妇女作为研究对象。所有研究对象行阴道镜及宫颈HPV检测。结果阴道镜对宫颈癌及癌前病变的阳性检出率为75.00%,而HPV对宫颈相关疾病的阳性检出率为70.92%,差异具有统计学意义(P<0.05)。阴道镜在宫颈癌及癌前病变的检测中的灵敏度(93.43%)以及准确率(59.26%)都优于HPV,差异具有统计学意义(P<0.05)。阴道镜和HPV联合检测对宫颈癌及癌前病变的诊断率为99.53%,显著高于阴道镜(93.43%)和HPV(85.45%)单独对宫颈癌及癌前病变的诊断率相比,差异具有统计学意义(P<0.05)。结论阴道镜对宫颈癌及癌前病变的检出率高于HPV检测,但阴道镜与宫颈HPV检测两者联合应用可以显著提高宫颈癌及癌前病变的阳性诊断率,临床诊断时应该根据患者的实际情况选择合适的方法。Objective To compare the diagnosis rates of colposcopy and cervical HPV testing for cervical cancer and precancerous lesions. Methods 540 cases of married women received screening of cervical lesions were the research object,all subjects underwent colposcopy and cervical HPV testing. Results The positive rate of colposcopy for cervical cancer and precancerous lesions was 75. 00%,and positive detection rate of HPV for cervical lesions was 70. 92%,the difference was statistically significant（ P〈0. 05）. Detection of colposcopy for cervical cancer and precancerous lesions in sensitivity（ 93. 43%） and accuracy（ 59. 26%） were superior to HPV,the difference was statistically significant（ P〈0. 05）. Detection rate of colposcopy combined with HPV in the diagnosis of cervical cancer and precancerous lesions was 99. 53%,which was significantly higher than colposcopy（ 93. 43%） and HPV（ 85. 45%） alone,the difference was statistically significant（ P〈0. 05）. Conclusion The detection rate of colposcopy for cervical cancer and precancerous lesions is higher than that of HPV,but the combined application of colposcopy and HPV can significantly improve the positive diagnosis rate of cervical cancer and precancerous lesions,we should choose the appropriate method according to the actual situation of patients.
