帕利哌酮与利培酮治疗阿尔茨海默病性痴呆的临床疗效比较

摘　　要：目的比较帕利哌酮与利培酮治疗阿尔茨海默病(AD)性痴呆的临床疗效。方法选择2013年6月至2015年6月荆州市精神卫生中心收治的AD性痴呆患者84例,按照随机数字表法分为帕利哌酮组和利培酮组,各42例。帕利哌酮组患者服用帕利哌酮进行治疗,起始剂量为3 mg/d,最大使用剂量为9 mg/d;利培酮组服用利培酮进行治疗,初始剂量为0.5 mg/d,最大剂量≤3 mg/d。比较两组患者的临床疗效及认知功能。结果帕利哌酮组有效率显著高于利培酮组[92.9%(39/42)比69.0%(29/42),P<0.05]。治疗后帕利哌酮组患者在治疗2、4、8周以及停药4周的TESS评分均较利培酮组低[(4.1±1.0)分比(5.3±1.3)分,(5.4±1.2)分比(6.8±1.2)分,(6.0±1.3)分比(8.3±1.4)分,(3.6±1.2)分比(5.3±1.1)分](P<0.01)。帕利哌酮组患者在治疗2、4、8周的简易智能量表(MMSE)评分均显著高于利培酮组[(22.7±2.2)分比(19.5±1.6)分,(25.3±2.7)分比(20.7±2.2)分,(28.4±3.1)分比(25.3±2.9)分](P<0.01)。结论相对于利培酮,帕利哌酮治疗AD引起的痴呆具有更加确切的临床疗效,安全性较高。Objective To compare the clinical efficacy of paliperidone and risperidone in treatment of dementia and psychiatric symptoms caused by Alzheimer disease （AD）. Methods Total of 84 AD patients treated in Jingzhou Mental Health Center from Jun. 2013 to Jun. 2015 were included in the study divided into paliperidone group and risperidone group according to the random number table method ,42 eases each. The paliperidone group was given, paliperidone treatment,with the initial dose of 3 mg/d and the maximum dose of 9 mg/d;the risperidone group was given rispefidone therapy, with the initial dose of 0. 5 mg./d and the maxi- mum dose ~〈3 mg/d. And the clinical efficacy,cognitive function of the two groups were compared. Results The total effective rate of the paliperidone group was significantly higher than the risperidone group [ 92. 9 % （ 39/42 ） vs 69. 0 % （ 29/42 ） , P 〈 0.05 ]. After 2 weeks,4 weeks, 8 weeks of treatment, and 4 weeks after treatment,the TESS score of the paliperidone group was significantly lower than the risperidone group [（4. 1 ±1.0） scores vs （5.3 ±1.3） scores vs （5.4 ±1.2） scores vs （6.8±1.2） scores, （6.0 ±1.3） scores vs （8.3 ± 1.4） scores, （3.6 ± 1.2） scores vs （5.3 ±1.1 ） scores ] （ P 〈 0. 05 ）. At the treatment of 2,4,8 weeks, the mini mentalstateexamination （MMSE） score of the paliperidone group was significantly higher than the risperidone group[ （22.7 ±2. 2） scores vs （ 19.5 ± 1.6） scores, （25.3 ±2.7） scores vs （20.7±2.2） scores,（28.4±3.1） scoresvs （25.3±2.9） scores]（P〈0.05）.Concluslon Compared with risperidone, paliperidone has more definite clinical effect for AD caused dementia and psychiatric symp- toms ,with higher safety.
