摘 要:目的探讨分析循环肿瘤细胞(CTC)在非小细胞肺癌患者中的诊断价值及其与临床病理特征的关系。方法选取60例非小细胞肺癌患者,40例慢性支气管炎患者及40例健康志愿者,Cellsearch法计数患者静脉血中CTC数目,分析其与性别、年龄、临床病理分期及肿瘤标志物的关系。结果非小细胞肺癌患者CTC数目显著高于慢性支气管炎患者和健康体检者(P<0.01);非小细胞肺癌患者外周血中CTC数目与性别、年龄、原发肿瘤分期无关(P>0.05);外周血CTC阳性率与非小细胞肺癌的淋巴结分期无明显相关性(P>0.05);转移性(M1)非小细胞肺癌患者CTC阳性率显著高于非转移性(M0)患者(P<0.05);不同TNM分期(Ⅰ~Ⅱ,III^IV期)患者CTC阳性率分别为42.9%,76.1%,差异有统计学意义(P<0.05)。结论非小细胞肺癌患者外周血中CTC数目与临床特征相关,通过检测CTC可初步反映疾病状态,并有望成为新的血液学指标来提高非小细胞肺癌的早期诊断。Objective To investigate the diagnostic value of circulating tumor cells (CTC) in non -small cell lung cancer and its relationship with clinical features.Methods Totally 60 patients with non-small cell lung cancer were selected for study,40 patients with chronic bronchitis and 40 normal volunteers were selected as the control groups.The CTC expression of peripheral blood was detected by CellSearch System, and its relationship with the age, sex, clinical stage and tumor marker was analyzed. Results CTC count was significantly higher in non-small cell lung cancer patients than in chronic bronchitis patients and healthy controls (P〈0.01). The number of CTC in peripheral blood has no association with the gender,age and the stage of the primary tumor(P〉0.05) .The positive rate of CTC in different lymph node staging has no significant cor- relation (P〉0.05),while the number of CTC of the metastatic (M1)non-small cell lung cancer patients is higher than the non metastasis (M0) (P〈0.05) ;The positive rate of TNM stage [ ~ lI and Ill^IV is 45.5% and 72.4 %, which different has statistically significance (P〈0.05);The positive rate of CTC was correlated with the CEA,CYFRA21-1 and SCC(P〈0.0S).Conclusion CTC counting in non-small cell lung cancer patients has certain correlation with clinical charac- teristics,can preliminarily reflect the patient's disease states,and be expected to become new hematology indexes to improve the early diagnosis of non-small cell lung cancer.
