透明角膜切口白内障超声乳化吸除术对白内障患者角膜屈光状态及视觉质量的影响

摘　　要：目的探讨行透明角膜切口白内障超声乳化吸除术对白内障患者角膜屈光状态及视觉质量的影响。方法选取白内障患者30例32眼,行透明角膜切口白内障超声乳化吸除术联合人工晶状体植入术,比较患者手术前后的角膜屈光状态及视觉质量,主要包括视力、角膜散光、屈光度、对比敏感度检查、眼压及角膜波前像差等。结果术后各时点的视力、角膜散光、屈光度、眼压较术前均有显著差异(P<0.05);术后各时点明视下的敏感度较术前无显著差异(P>0.05),术后3.0及18.0处暗视下的敏感度较术前显著升高(P<0.05);患者术后1周及1月的高阶像差、三阶像差、彗星高于术前(P<0.05);术后各时间点的四阶像差、球差较术前均有提高(P<0.05)。结论透明角膜切口白内障超声乳化吸除术联合非球面折叠式人工晶体的植入具有早期改善视觉质量,提高视力,稳定患者的角膜屈光状态及改善眼压等优点,值得临床推广使用。Objective To discuss the influence of phacoemulsification on the refractive status and visual quality of patients with cataract.Methods 30 patients （32 eyes） undergoing phacoemulsification and foldable lens implantation through upper corneal echelon incision were chosen. The refractive status and visual quality of patients were compared between before and af- ter operation, including visual acuity,corneal astigmatism, diopter,contrast sensitivity, intraocular pressure, corneal wavefront aberration and so on.Results The postoperative visual acuity （visual acuity, corneal astigmatism, diopter, intraocular pressure） was significantly better than preoperative （P〈0.05） ,4 order aberration and spherical aberration were higher than the level at all periods after surgery.Conclusion The phacoemulsification can improve patients＂ corneal astigmatism and visual quality with a lot of promotion prospects.
