MRI小肠造影诊断小肠肿瘤性疾病的价值

摘　　要：目的探讨MRI小肠造影诊断小肠肿瘤性疾病的临床价值。方法疑诊小肠肿瘤性疾病患者108例,于MRI扫描前60min开始分4次口服质量分数2.5%等渗甘露醇溶液2 000mL,观察十二指肠、空肠和回肠病变累及状况;以活检或手术组织病理结果为金标准,计算MRI小肠造影诊断小肠肿瘤性疾病的敏感度、特异度、阳性预测值、阴性预测值。结果 MRI小肠造影诊断小肠腺癌27例,间质瘤25例,淋巴瘤23例,脂肪瘤6例,余27例未见异常;病灶位于十二指肠8例,空肠40例,回肠33例;内镜下活检组织病理显示20例未见异常,余88例诊断小肠肿瘤性疾病者均行手术治疗,术后组织病理证实小肠腺癌29例、间质瘤28例、淋巴瘤25例、脂肪瘤6例,病灶位于十二指肠8例、空肠43例、回肠37例;与组织病理结果进行对照,MRI小肠造影诊断小肠肿瘤性疾病的敏感度为92.1%,特异度为100.0%,阳性预测值为100.0%,阴性预测值为74.1%。结论 MRI小肠造影在小肠肿瘤性疾病诊断中具有较高价值。Objective To explore the clinical value of MRI small bowel enema to the diagnosis of small intestinal tumors. Methods A total 108 cases of suspected small bowel tumors received oral administration of 2.5 % osmotic mannitol for 4 times with the total dose of 2 000 mL 60 min before MRI scan to observe the duodenum, jejunum and ileum lesions. The histolpathoiogical results were as the gold standard to calculate the sensitivity, specificity, positive predictive value and negative predictive value of MRI small bowel enema to the diagnosis of small intestinal tumors. Results MRI small bowel enema found 27 cases of small intestinal adenocarcinoma, 25 cases of stromal tumor, 23 cases of lymphoma, 6 cases of lipoma, and 27 normal cases. The lesions were located at the duodenum in 8 cases, at the jejunnm in 40 cases and at the ileum in 33 cases. The endoscopic histopathologic results confirmed normal in 20 cases and the other 88 cases were confirmed small bowel tumors and underwent operation. The postoperative histopathologic results confirmed 29 cases of small intestinal adenocarcinoma, 28 cases of stromal tumor, 25 cases of lymphoma and 6 cases of lipoma. The lesions were located at the duodenum in 8 cases, at the jejunnm in 40 cases and at the ileum in 33 cases. Compared with the histopathological findings, the sensitivity of MRI small bowel enema for the diagnosis of small intestinal tumors was 92.1%, the specificity was 100.0%, positive predictive value was 100.0%, and negative predictive value was 74.1%. Conclusion MRI small bowel enema has a great value to the diagnosis of small intestinal tumors.
