摘 要:目的探讨肠内营养对急性脑卒中伴吞咽障碍患者短期预后的影响。方法将86例急性脑卒中伴吞咽障碍患者随机分为肠内营养组(实验组)43例和家庭传统喂养组(对照组)43例。比较21d后2组患者的营养状态、感染发生率、神经功能缺损评分(NIHSS)。结果住院21d时,实验组血清白蛋白、前白蛋白、血红蛋白、上臂肌围等指标均优于对照组,差异均有统计学意义(P<0.05);实验组合并感染的发生率为27.9%,对照组为53.5%,2组差异有统计学意义(P<0.05);实验组NIHSS评分(11.13±2.03)分低于对照组的(15.35±2.76)分,2组间比较差异有统计学意义(P<0.05)。结论对于急性脑卒中伴吞咽障碍患者,早期实施肠内营养能够提升患者营养状态,减少感染发生率,降低神经功能缺损评分,从而改善患者短期预后。Objective To investigate the effect of enteral nutrition(EN) on short-term prognosis m acute stroke patients with dysphagia. Methods Totally 86 acute stroke eases with dysphagia were divided into study group and control group,43 ca- ses in each group. The study group received enteral nutrition and the control group received traditional family feeding. Then pa- tients' nutritional status,incidence of infection, and the national institute of health stroke scales(NIHSS) were compared in the two groups on the 21st day. Results On the 21st day, the nutritional parameters, such as serum albumin, prealbumin, haemoglo- bin,upper-arm muscular circumference in the study group, were better than those in the control group with statistical differ-ences(P〈0.05). The concurrent infection rate was 27.9%in the study group and 53.5% in the control group,and the differ- ence between the two groups was statistically significant(P〈0. 05). In addition, the NIHSS scores in the experimental group (11.13±2.03) were significantly lower than those in the control group(15.35±2.76)(P〈0.05). Conclusion For acute stroke patients with dysphagia,earIy enteral nutrition could improve the nutritional status, reduce the incidence of infection and im- prove the neurological deficit scores as well as the short-term prognosis.
