选择性肝动脉化疗栓塞术联合经皮微波消融治疗大肝癌的临床观察

摘　　要：目的探讨选择性肝动脉化疗栓塞术联合微波消融治疗大肝癌的临床效果。方法选取本院2011年6月~2014年6月收治的56例大肝癌患者作为研究对象,按照患者及家属意愿分为观察组（30例）和对照组（26例）。观察组给予TACE联合PMCT治疗,对照组给予单纯TACE治疗。比较两组治疗前后的血液学指标以及近期疗效、远期生存率。结果观察组术后的AFP水平显著低于治疗前及对照组术后,差异有统计学意义（P〈0.01）。观察组术后的肿瘤最大径显著小于治疗前及对照组术后,差异有统计学意义（P〈0.05）。观察组的近期临床有效率为86.7%,显著高于对照组的57.7%,差异有统计学意义（P〈0.01）。观察组的6、12、18个月生存率显著高于对照组,差异有统计学意义（P〈0.05）。两组的不良反应多轻微,经保守治疗后均恢复正常。结论 TACE联合PMCT能够提高大肝癌患者的近期疗效及远期生存率,且安全性好,值得临床推广应用。Objective To explore the effect of selective hepatic artery chemoembolization combined with percutaneous microwave coagulation therapy（PMCT） in the treatment of large hepatocarcinoma. Methods 56 patients with large hepatocarcinoma in our hospital from June 2011 to June 2014 were selected and divided into the observation group（30 patients） and the control group（26 patients） according to the wishes of patients and their families.The observation group was treated with TACE combined with PMCT,the control group was treated with TACE.The blood indicator before and after treatment,the short-term efficacy and the long-term survival rate in the two groups was compared. Results The level of AFP in the observation group after operation was lower than that before operation and in the control group after operation,with significant difference（P〈0.01）.The maximum diameter of the tumor in the observation group after operation was less than that before operation and in the control group after operation,with significant difference（P〈0.05）.The recent clinical effective rate of the observation group was 86.7%,which was higher than that of the control group 57.7%,with significant difference（P〈0.01）.The survival rate of the observation group in 6,12 and 18 months was higher than that of the control group,with significant difference（P〈0.05）.The adverse reactions of the two groups were slightly,after conservative treatment,all returned to normal. Conclusion TACE combined with PMCT can improve the short-term efficacy and long-term survival rate of patients with large hepatocarcinoma,and the safety is good,it is worthy of clinical promotion and application.
